Background and objectiveImplantable venous-access ports(IVAP)is a venous access device that can be totally implanted.IVAP is widely used in patients with diseases such as cancers and chronic digestive diseases which requires long-term intermittent intravenous therapy.By minimizing the need for frequent vascular access,IVAP can guarantee the safe infusion of chemotherapy,improve the patients' quality of life and reduce the risk of infection.Now,the standard procedure of puncture in the implantation of IVAP is to puncture at first and then cut the skin.In this study,we aimed to analyze the difference between puncture before cut method and cut before puncture method and to provide evidence in the improvement of the implantation of venous-access ports.Subjects and MethodsIn this study,participants were included from department of breast surgery,Qingdao Central Hospital.All the participants were patients with breast cancer confirmed by pathological examination.Demographic information was collected by the electronic medical record system.Venous-access ports were implanted before chemotherapy.The participants were randomly assigned in to two groups.Participants in experimental group were received cut before puncture method and participants in control group were received puncture before cut method in the puncture procedure of implantation of venous-access ports.The difference between two puncture methods were assessed by lengths of cut,procedure time,amount of bleeding,pain after procedure and the degree of satisfaction of patients' appearance and movement after procedure.Wilcoxon test was used to compare intergroup difference for continuous variables,while Chi-squared test was used to examine categorical variables.ResultsA total of eighty patients with breast cancer were included in our study.There were forty participants in each of the experimental and control groups.The mean age of participants were 53.49 years old.There were no differences in age,site of puncture between two groups.The puncture methods have no influence on lengths of cut,procedure time and pain after procedure.However,we have found that the amount of bleeding is significantly lower in experimental group than that in control group(p=0.04).Besides,the degree of satisfaction of appearance and movement is significantly higher in experimental group than those in control group.And the experimental group had higher satisfaction rate of appearance than control group(p=0.05).ConclusionPuncture methods had no influence on lengths of cut,procedure time and pain after procedure.But the cut before puncture method can reduce amount of bleeding and improve patients' satisfaction of appearance and movement.
