Objective : To explore the relationship between angiotensin II level and serum adiponectin in essential hypertension patients,and its clinical significance.Methods:The essay selects a total of 280 patients who were admitted to the Second Comrade Department of the First Affiliated Hospital of Xin jiang Medical University from May2016 to December 2018 as the research object.According to diagnostic basis of essential hypertension in 2018 Chinese Guidelines for the Management of Hypertension,they are divided into hypertensive group 221 cases and control group 103 cases.The hypertensive group is divided into normal hypertensive treatment group 123 cases and untreated hypertensive group 98 cases according to the treatment situation.The control group is divided into normal blood pressure group 54 cases and normal high blood pressure group 49 cases based on different blood pressure levels.The essay collects relevant medical history,clinical data,and detects the levels of serum APN and Ang? by ELISA.It analyzes the changes of serum APN and the correlation between APN and Ang? among the four groups.Result:(1)The clinical features of normal BP group and normal high BP group were compared,the blood glucose,24 SBP,24 DBP,24 MAP and Ang?in the normal high BP group were higher than the normal BP group,P<0.05,the difference had statistical significance.And serum Cys-C was lower than the normal BP group,P<0.05,the difference had statistical significance.The comparison conducted between age,glycosylated hemoglobin(HBALC),BUN,serum Cr,LDL-C and HDL-C had no statistical significance(P>0.05).(2)The clinical features of normal hypertensive treatment group and untreated hypertensive group were compared,the indicators FBG,TG,TC,24 SBP,24 DBP,24 MAP,and Ang ?of hypertensive treatment group were lower than the untreated hypertensive group,P<0.05,the difference had statistical significance.The serum APN level in hypertensive treatment group was higher than the untreated group,P<0.05,the difference had statistical significance.The comparison conducted between age,BMI,glycosylated hemoglobin,BUN,serum Cr,LDL-C and HDL-C had no statistical significance(P>0.05).(3)The levels of APN and Ang ? in the four groups were all P < 0.05,and the difference was statistically significant.APN level: The normal blood pressure group was higher than the other three groups,the untreated hypertensive group was lower than the other three groups,and the hypertensive treated group was higher than the normal high blood pressure group.Ang? level: The normal blood pressure group was lower than the other three groups,the untreatment hypertensiv group was higher than other three groups,and the hypertensive treatment group was higher than the normal high blood pressure group.(4)In the normal high BP group,the female APN level was higher than that of male group,P<0.05,the difference had statistical significance,in other three groups,the comparison between male and female had no statistical significance.The APN level among male and female patients decreased with the rising of mean arterial pressure,while treatment could raise the APN level.(5)APN of all patients were analyzed,and it had a negative association with BMI(r=-0.144,P<0.05),24 SBP(r=-0.367,P<0.05),24 DBP(r=-0.330,P<0.05)and 24 MAP(r=-0.371,P<0.05),there were statistical significance.Correlation analysis was conducted between APN level and Ang ?level,and the result indicated that plasma APN had a negative association with Ang ?(r=-0.234,P<0.05).(6)In the control group,APN reflected negative association with uric acid and FBG,it had a positive association with HDL,with both P<0.05,there was statistical significance.In the correlation analysis,the variable P<0.10 was included into multi-factor regression analysis,and the result indicated that 24 MAP is the inhibited factor of APN.In the hypertensive group,APN had a positive association with age,HDL and serum Cys-C,with all P<0.05,there were statistical significance.In the correlation analysis,the variable P<0.10 was included into the multi-factor regression analysis,and the result indicated that age and HDL were the stimuli of APN,the mean arterial pressure was still the inhibited factor.Conclusion: The APN level among primary hypertension has apparent decrease and the APN level shall decrease with the rising of mean arterial pressure,while the treatment can rise the APN level.The APN level has a negative association with the level of Ang ?.
