Objective: A retrospective study was conducted to explore the clinical features and characteristics of TCM syndrome in patients with no fever symptoms,and to provide theoretical basis for the realization of accurate syndrome differentiation.Methods:Selected by ward electronic medical record system in January 2013-August 2017 clear diagnosis of CAP and check in affiliated hospital of shandong university of TCM lung disease patients,according to the presence of fever will be divided into the heat group and no fever.Patients basic information collected through electronic medical record system,seasons,time,length of hospital stay,and confirmed the clinical symptoms and signs,clinical laboratory index(blood routine,c-reactive protein and blood sedimentation,sputum culture)distribution characteristics,imaging,clinical data and fill in the observation scale,using SPSS20.0 software,the clinical data of frequency statistics and chi-square test,to observe the clinical data and the characteristics of TCM syndrome factor.Results:1.In recent 5 years,the proportion of patients with no fever CAP in our hospital has been increasing.2.The patients with febrile CAP were more common in middle and old age(74.4% of patients over 50 years old)and female patients(64.0%),and the incidence was more frequent in winter and spring.76.7% of the patients had no history of smoking,and 74.4% had basic diseases.Among them,respiratory diseases,digestive system diseases,circulatory system diseases are common.3.The clinical diagnosis time was more than 1 week,and 23.3% of the patients were diagnosed after 30 days,and the number of hospitalized days was more than 15 days.Clinical symptoms as cough,sputum,chest tightness,shortness of breath,suppress asthma and other respiratory symptoms,some patients with lack of power,flustered,depression,nausea,anorexia,abdominal discomfort,anti acid wait for a symptom.The total number of peripheral blood cells(86.1%),the percentage of neutrophils(75.6%),and CRP count(60.5%)were in normal range,but 74.5% of the patients had increased ESR.Imaging examination,the lesion is seen in double,lower lung field.4.The syndrome differentiation of Chinese medicine is mainly based on the fact that the tongue is red or reddish,and the tongue is white and yellow.The pulse of the pulse and the number of veins,the pulse,and the pulse of the pulse.The common position syndrome is lung,spleen and kidney.Common syndromes are qi deficiency,phlegm(wet),heat,Yin deficiency,blood stasis;Often witness element combination for lung and spleen deficiency phlegm heat accumulate lung syndrome,lung spleen and kidney Yin deficiency of phlegm heat accumulate lung,lung deficiency phlegm wet temper aggregates lung,pulmonary lung,lung deficiency phlegm and blood stasis resistance temper temper virtual wind hot/cold make the lung,lung spleen and kidney Yin deficiency lung phlegm and blood stasis resistance.Conclusion:In view of the special clinical features of CAP patients with no fever,In clinical practice,it is important to be aware of the possibility of pneumonia in the following situations: Chronic unexplained lack of fatigue,panic,malaise,nausea,acid,sodium,abdominal discomfort,or the underlying disease was aggravated.Patients with normal temperature,normal blood routine or CRP count should routinely perform chest X-ray or CT examination to avoid misdiagnosis and delay of treatment.
