Objective:To probe into the causes and treatment of nonunion fracture of long bone of extremities,and to design a personalized strategy to improve the curative effect.Method:From March 2016 to Apr 2017,55 patients who suffered from the uninfected postoperative fractures nonunion were treated in Shandong University of TCM Affilliated Hospital Orthopaedics Patients' Room.55 patients met the inclusive criteria,Being directed against the causes for the ununion,There are several therapeutic methods we adopted: replacing internal fixation plus bone grafting,reserving original fixation and using simple bone grafting and giving local biological stimulation with traditional Chinese medicine,in the end,we collect the clinical data and analyse them in a retrospective way.Results:all the 55 patients were followed up completely,and the follow-up results showed that 94.4% of the patients received an average of 3.5 months(3-10 months)of fracture healing.Two cases were not healed again because of internal fixation failure.According to the corresponding scoring criteria,the adjacent joints of patients who had been recovered from the fracture were scored from function,activity pain and activity degree.Among them,49 cases were excellent,4 cases were good,and 2 case were middle,the excellent rate was 89%.Conclusion:Iatrogenic factors are the main causes of fracture nonunion after operation,according to the specific reasons for personalized surgical design,Treatment of fracture end carefully during operation,Taking effective and reliable internal fixation and effective infection controlling,autologous bone grafting and taking the advantages of TCM are effective measures for improving the curative effect of fracture.
