Analysis Of TCM Syndrome Factor In Elderly Patients With Multimorbindity

Objective: To explore the distribution patterns of elderly inpatients with multimorbindity in the number and type of diseases.To explore the distribution patterns of the TCM syndrome factor in elderly inpatients with multimorbindity.To explore the correlation between different diseases and syndrome factors in elderly inpatients with multimorbindity.To provide reference for the application of TCM in the management and prevention and treatment of elderly multimorbindity.Methods: The clinical data of inpatients in the Department of Geriatrics,Affiliated Hospitional of Shandong University of TCM from April 2016 to October 2017 were collected,the syndrome factors were extracted,and the data were statistically analyzed by SPSS and analyzed retrospectively.Result: 1.A total of 252 elderly inpatients with multimorbindity were collected,with an average of(80.56±8.69)years,with 136 males and 116 females.The number of their diseases ranged from 2 to 14.72.6% of them had 4 to 9 types of diesases at the same time.The prevalence of different types of diseases in the elderly inpatients from high to low were that coronary heart disease,hypertension,cerebrovascular disease,type 2 diabetes,arrhythmia,osteoarthropathy,infectious diseases,chronic respiratory disease,lower extremities vascular diseases,gastrointestinal diseases.Compared with elderly inpatients in 60-79 years,Coronary heart disease,cerebrovascular disease,arrhythmia,infectious diseases and lower extremity vascular disease was more common in elderly inpatients ?80 years(P<0.05).2.According to the frequency,the syndrome factor on location of diseases in elderly inpatients in turn was that kidney,lung,brain,heart,meridians and collaterals,spleen,liver,stomach,bones,urinary bladder.According to the frequency,the asthenic syndrome and sthenic syndrome factor in turn was that qi asthenia,yang deficiency,blood asthenia,yin deficiency,phlegm,dampness,qi stagnation,blood stasis,retained fluid,fire(heat),endogenous wind.Compared with elderly inpatients in ?80 years,the syndrome factor liver was more common in elderly inpatients 60-79 years(P<0.05).Compared with elderly inpatients in 60-79 years,the syndrome factor kidney,lung,brain,meridians and collaterals were more common in elderly inpatients ?80 years(P<0.05).Compared with the male elderly inpatients,the syndrome factor liver was more common in the female elderly inpatients(P<0.05).Compared with the female elderly inpatients,the syndrome factor meridians and collaterals was more common in the male elderly inpatients(P<0.05).3.Compared with elderly inpatients without angina pectoris,the syndrome factor of heart and qi asthenia were more common in the elderly inpatients with angina pectoris(P<0.05).Compared with elderly inpatients without cerebrovascular disease,the syndrome factor of meridians and collaterals,brain,phlegm were more common in the elderly inpatients with cerebrovascular disease(P<0.05).Compared with elderly inpatients without infectious diseases,the syndrome factor of yin deficiency,phlegm,blood stasis,retained fluid,lung,brain,meridians and collaterals,spleen were more common in the elderly inpatients with infectious diseases(P<0.05).Compared with elderly inpatients without lower extremities vascular diseases,the syndrome factor of blood asthenia were more common in the elderly inpatients with lower extremities vascular diseases(P<0.05).In addition,there were more syndromes in the patients with infectious diseases or atrial fibrillation(P<0.05).Conclusion: There are several diseases in the elderly patients with multimorbindity.The distribution of the type in disease will change a certain amount with age.Therefore,the health management of elderly patients with multimorbindity need more timely assessment in order to select the optimal program.The clinical manifestations and syndromes are more complicated in elderly patients with multimorbindity.It is more difficult to differentiate syndrome and to establish the therapeutic principle for them.The pathological changes of multimorbindity belongs to mixture of asthenia and sthenia.Some syndromes are specific in elderly patients with some diseases.It is necessary to consider comprehensively for syndrome differentiation,and to choose a reasonable medication compatibility,when applying TCM in elderly patients with multimorbindity.
