The Physician Scheduling Method Of Outpatient Department In Specialized Hospitals Considering Working Across Departments

In recent years,with the improvement of medical service level and the expansion of people's service demand,China's medical service industry has developed rapidly.Both in the national fiscal expenditure and the household consumption expenditure,the medical expense proportion is high.The plan for deepening the reform of the medical and health system in the 13th five-year plan has been clearly put forward that "To 2020,we should generally establish the more perfect public health service system and medical service system,the more sound medical security system compares a standard,the more normative drug supply security system and comprehensive regulatory system,and the more scientific management system and operational mechanism of medical and health institutions.”It can be seen that the transformation of medical services to a scientific way is urgent.As an effective complement and rear support to general hospitals,the development of specialized hospitals cannot be ignored.Under the background of shortage of medical resources,specialized hospitals are faced with such special problems as single disease,large number of outpatients and high mobility of patients.For specialized hospitals,such as the stomatology hospital,the eye hospital and the dermatological hospital,the outpatient service takes up the majority of the whole diagnosis and treatment process,which means that a good outpatient service management system directly determines the operation efficiency of the outpatient department of the hospital,and directly affects the service quality of the whole hospital.And physician,as the dominant and expensive resource in the diagnosis and treatment work,becomes the top priority in the outpatient work.In real life,many hospital physicians scheduling system is lack of rationality and scientific nature,mostly relies on experience of administrators.At the same time,the fixed mode of physicians scheduling system can not meet the actual requirements of patients in different period of time.It not only can cause the hospital internal congestion phenomenon,more seriously can make patients with anxiety,doctor-patient conflicts that cannot be forecasted.Based on the reality of the background,this article is from the perspective of hospital outpatient department managers,through the field research of Dalian dermatology hospital.The physician scheduling problem is our research object,and the background of the reality of the hospital labor laws and regulations is fully taken into account.When the personal information,frequency division mode and the physician and patient distribution is known,respectively,we establish the following two model is.One is the scheduling model of outpatient physicians in specialized hospitals for a single department,that is,the scheduling model for each physician in the case of a certain number in the department and no influence on each other.It aims at satisfying the personal preference of each physician of different professional levels for shift as much as possible,ensuring the basic operation of the department under the premise of ensuring the completion of patient diagnosis and treatment services,thus establishing a linear integer programming model.The actual data of Dalian dermatology hospital were used to carry out the solution,the experimental results were analyzed,the feasibility of the model was verified,and the sensitivity was analyzed.The second is the interdisciplinary outpatient scheduling model of specialized hospitals,which allows the bottleneck of outpatient department to transfer physicians to meet the needs of more patients.At this point,there is interaction between each department.Under the condition of the requirements of patients and the regulations of the hospital are met,the integer programming model of scheduling method is generated,and the data is introduced to solve and analyze the experimental results.On account of the actual needs of the whole specialty hospital industry and the special scene of the outpatient department of the specialty hospital,this paper summarizes the unique characteristics of the specialty hospital under the support of historical data and field research,generates scientific problems,establishes reasonable mathematical models and applies cplex programming to solve problems.The experimental results verify the reliability of the model,and prove that the model can improve the work efficiency of the outpatient department and relieve the pressure of diagnosis and treatment in the outpatient department.
