Background National maritime disputes in recent years,local war,sea confrontation is also growing,the Taiwan strait and the east China sea,the south China sea knight,in time of peace prepare for war,thought is be prepared,military medical support mission is very important,no matter in peaceful environment or in the war,the blast injury caused by a large number of casualties were reported from time to tome,various acute injuries also happens quite often,no matter what the cause of explosion injury,can cause a direct damage to the liver tissue and inflammatory markers or explosion shock wave after the injuries caused by the implosion effect and indirect damage caused by the action of the waterfall effect,its high mortality rate.Underwater explosion is under the certain conditions,especially the main sea war injuries,but because of the confidentiality of such research at home and abroad,access to the study of underwater blast injury data scarce,therefore it is necessary for this type of damage to related research.The liver is an organ that is prone to injury when the abdomen is damaged,and the hemorrhage,shock and numerous complications caused by liver injury seriously endanger the patient's life.The mortality rate of liver injury can be as high as 14% ~40%.However,there is a lack of relevant research on the liver injury caused by underwater explosion,which is more important to carry out relevant basic and clinical research.Use of ustc underwater explosive devices of the department of modern force of underwater explosion induced liver injury model was established,and on this basis,further reveals the underwater explosion induced liver injury pathological mechanism and treatment method after injury.Therefore,the diagnosis,first aid and treatment of the damage caused by the underwater explosion shock wave will be one of the key points in the study of military medicine and trauma surgery.The research we have done is to explore and search for the mechanism of liver injury induced by underwater explosion and the early treatment.Research purpose By what we have done experiments reveal the underwater explosion induced acute liver injury mechanism,to explore the xuebijing of underwater explosion rabbit acute closed hepatic injury,the effect of early intervention study for clinical treatment of liver injury caused by underwater explosion provide certain reference.Materials and methods Health New Zealand rabbits were randomly divided into control group,damage group(only)and intervention group(after the explosion using xuebijing intervention),with underwater explosion to rabbit acute closed hepatic injury,enzyme league immune method to detect after the explosion for 0,4 hours,12 hours,24 hours a day rabbit serum ALT,concentration of IL-6,MMP-9,24 hours after the explosion experimental rabbit bleed to death,taking the liver pathology and tissue homogenate.In the 24 hours after the injury,the pathological morphological changes of the liver tissues of New Zealand rabbits were observed,and the changes of MMP9 were detected by fluorescence quantitative PCR and Western Blot,and the results were statistically analyzed.Results 12 hours after the explosion,the ALT expression level was significantly decreased in the 24-hour xuebijing group and the injured group,and the difference was statistically significant(P < 0.05).4 hours after explosion xuebijing amount of the expression of MMP-9 and damage group compared with the difference is not obvious(P > 0.05),12 hours after the explosion,24-hour xuebijing group and injury group compared with the expression of MMP-9 quantity significantly reduced,statistically significant difference(P < 0.05).4 hours after explosion xuebijing the expression of IL-6 and damage group compared with the difference is not obvious(P > 0.05),12 hours after the explosion,24-hour xuebijing amount of IL-6 expression and damage group,compared statistically significant difference(P < 0.05),24 hours after the explosion experimental rabbit bleed to death,MMP-9 mrna expression of the liver tissue injury group was higher than normal control group and the xuebijing treatment group,the difference was statistically significant(P < 0.05).The lesion of liver disease was obvious,obvious edema and hemorrhage were visible,and the pathological damage was improved.Conclusion xuebijing under the intervention experiment rabbit in 12 hours,24 hours after the explosion in serum ALT,MMP-9,IL-6 significantly reduced,explosion roup of MMP-9 mrna expression in liver tissue increased obviously,and the xuebijing treatment group is relatively low,after the explosion,liver gross and microscopic observation showed that the damage degree of xuebijing treatment group significantly reduce in the explosion group,showed the xuebijing to underwater explosion to rabbit acute closed hepatic injury has a protective effect.
