Risk Factors Associated With Early Implant Failure

Aim:The retrospective study assessed the risk factors associated with early implant failure at the patient and implant based analysis.Materials and Methods:A total of 18,199 patients received 30,959 dental implants who underwent dental implant surgery in the Zhejiang Hospital Affiliated Hospital and Hangzhou Hospital Affiliated during the years of 2011-2015.Age,gender,jaw,location,implant brands,implant length,implant diameter,bone augmentation procedures and the number of implants placed per patient were recorded.A multivariate generalized estimating equation(GEE)logistic regression was used to identify risk factors related to early implant failure.Results:The implants between January 2011 to December 2015 was collected,the deadline for observation is December 2016.Finally,the cumulative survival rates were 98.0%for patients and 98.7%for implants after 0-72 months observation time.A total of 183 patients with 194 implants were lost before or at the abutment connection,with a early failure rate of 0.6%at the implant level and 1.0%at the patient level.Based on the above data,the multivariate GEE logistic regression showed that:1.The early failure rate of male patients was significantly higher than that of female patients(p<0.1 OR=1.97).2.The risk of early failure in patients aged 41 to 60 years was significantly higher than that in patients aged less than 40 years(p<0.1 OR=1.68),but the patients older than 60 years old had more higher early failure rate(p<0.1 OR=1.97).3.Compared with other regional implants,the early failure rate of mandibular anterior dental implants increased significantly(p<0.1 OR=2.17).Conclusions:General factors such as male,elderly patients,mandibular anterior location were associated with early implant failure.Therefore,we should inquire about the past history and evaluate the risk factors carefully before the operation,especially for the elderly patients,male and the mandibular anterior teeth,so as to strictly grasp the indications and improving the success rate of oral implants.
