[Objective]Evaluate the prosthetic result of the different implantic addition operation of the 10-year clinical application of the juncturae ossium implant. Analysis the effect of the retention rate of the different implant system, of the different operation and other factors in the long-term effect of the implant denture.[Method]1. 1473 implants had been implanted in 856 patients in the implant center of the Stomatology Hospital of Shandong University from the June of 1997 to the December of 2006. 120of them was BRANEMARK,20 of them was ITI, 640 was REPLACE, 132 was LIFECORE and 471 was BLB and 90 of them was made in china.2. Besides the common operation, we use different biomaterials (multiple bone inorganic materials, biofilm can be absorbed, Ti film can not be absorbed ) and addition operations (bone split, sinus lift, autogenous bone grafting ),when the bone width or the bone height is not enough, to increase the indication of implant, to raise the achievement ratio of implant.3. All the implants had been prosthesis before June of 2006, the average time of follow-up is 10-125 month. When the osseointegation of the implants is reached, we use the solid and the angle abutments to make the single crown, the bridge, removable full denture.4. Use X-ray, probing and other clinical examination methods to evaluate the stability, the function, the aesthetic effects, the shape of the tissue and the bone absorption of the implants. Use the Wheeler retention rate standard to evaluate, and use the Kaplan-Meier retention curve to analysis.[Results]1. Up to July of 2007,168 patients had been tracked, the proportion was 11.41%. 62 patients mailed their x-ray film, the proportion was 4.21%. the defaulting was 1243, the proportion was 84.39%.2. After the implant prosthesis had been completed 10 years, there are 1442 implants retented in 856 patients. There were 31implants fell off. 30 implants had been implanted again successfully, and only one case failed again.3. The follow-up revealed 7 implants had periimplantit in 3 patients. Two patients asked for making new PFM. Three crew fixed implants' crown appeared turning, after the crew cinch. Five patients did not satisfied about the color and the shape of the crown. But they all felt satisfied after making the new PFM.4. Bone split open was 168cases, bone pressurization was 69 cases, sinus lift was 167 cases, mental region bone transplantation was 54cases, flank bone transplanted was 18 cases and GTR 331 cases in the bone increment operation cases had been done addition operation. The operation offered good condition for the implants, and the long-term effect is good.5. The implant systems had different shape and surface had no different in longterm retention rate.6. According to the Wheeler retention standard, the 10-year retention rate was 95.23%.[Conclusion]1. All the implant system show good long-term effectiveness and persistence value in long-term clinic application. And there was no difference among them. The 10-year retention rate in the bone increment technical application was 95.2%.2. The bone transplant and the application of the biomaterial extend the indication. They all have ideal effectiveness and high long-term persistence value.3. In the review investigation, we discovered that the falling off of the implants before prosthetic was because no enough cooling, after prosthetic was because that there was caused by the inflamm around the implant and laterad or the justo maior bite force.4. The merit of the case review was the amount of cases was large, the time was long. Syst review investigate the long-term effectiveness and the retention rate in bone increment application for ten years. the defect was that the patients'dependence was bad, and the return visit rate was low. And some important index constrained by the condition. For example, the condition around the implants and the bone resorption can not get the objective statistics data.
