Objective:To evaluate the effects of aerobic exercise guided by mobile-phone-pedometer and WeChat,mainly on physical and psychological outcomes and quality of life of gynecologic oncology women undergoing chemotherapy in a randomized controlled trial.Methods:A total of 111 gynecological malignant tumor patients,admitted in a tertiary Women's and children's hospital in Ningbo city from September 2015 to September 2017,were randomly assigned to the control group(n=56)and the experimental group(n=55).The subjects in control group received routine care including psychological counseling,diet guidance,chemotherapy related instruction and online follow-up after discharge.In addition,the experimental group started to receive a 6-month mobile pedometer based aerobic exercise program at the admission of the second chemotherapy,requiring patients to have at least 150 minutes of low-medium intensity aerobic exercise per week.During the study period,2 patients died and 1 patient dropped out due to her migration in the control group while 3 patients withdrew due to exercise intensity insufficient and 2 patients dropped out due to chemotherapy interrupted in the experimental group.The final enrolled number of experimental group was 50 cases and the control group was 53 cases.Patients' height,body weight,walking steps per day,energy consumption were collected and psychological outcomes were assessed by self-rating anxiety scale(SAS)and self-rating of depression(SDS),and quality of life was assessed by European Organization for Research and Treatment of Cancer(EORTC QLQ-C30)in two groups on the time point of prior to the second chemotherapy and the sixth month after intervention.Data were analyzed by t-test and chi-square analysis using SPSS22.0,and the significant difference was set atp<0.05.Results:There were no significant differences in baseline of the two groups,including age,height,weight,body mass index,education level,marital status,caregiver,family monthly income,payment methods,tumor types,and disease stages(p>0.05).In the experimental group,the average amount of walk steps per day increased from 4746.50(3685.00,6348.50)to 8462.50(7153.25,10007.50)before and after 6 months intervention with statistical significant difference(p<0.001);Energy consumption also increased from(196.42±72.84)kcal to(275.48±66,49)kcal before and after 6 months intervention showing a significant difference(p<0.001).Meanwhile,the body weight of the experimental group decreased from(63.43± 11.36)kg to(58,92±8.26)kg and the BMI decreased from(24.76±3.99)kg/m2 to(23.02±2.88)kg/m2 with statistical significance before and after 6 months intervention(p<0.05);The body weight and the BMI were significantly lower in experimental group than those in control group(58.92±8.26 VS 62.66± 10.37,p<0.05 and 23.02±2.88 VS 24.58±4.44,p<0.05);Both the scores of SAS and SDS scale were significantly lower in experimental group than those in control group(60.58+11.20 vs65.83110.35,p<0.05 and 60.22±5.20 vs 63.77±9.62,p<0.05);Overall score of EORTC QLQ-C30 was(35.12±9.60)in experimental group and(31.71±6.16)in control group.Significant difference existed between two groups(p<0.05);With the exception of the cognitive item,score of function subscale of the EORTC QLQ-C30 in experimental group was higher than those in control group(p<0.05).The symptom scale(fatigue,nausea and vomiting)of EORTC QLQ-C30 in experimental group scored lower than those in control group with statistical significance(p<0.05).The specific items of the EORTC QLQ-C30 scored significantly lower than that of control group in all the items except for dyspnea and economic difficulties(P<0.05).Conclusions:Our study suggested that mobile-phone-pedometer and WeChat guided aerobic exercise can help increase the average daily amount of exercise for gynecological malignant tumor patients with chemotherapy,thus increasing the energy consumption.It can improve body physiological indicators and optimize psychological outcomes.Mean while,mobile-phone-pedometer and WeChat guided aerobic exercise can also improve the quality of life of chemotherapy patients with gynecological malignant tumors.
