ObjectiveIn this study, chondromalacia patellae was treated on the basis of oral medications with exercise therapy that increased the exercise of femoral adductor muscles. We investigate the effect of exercise therapy with increased adductor muscles exercise on the treatment of chondromalacia patellae by the comparison of the observation indexes before and after the exercises of the patients.Methods66 patients with chondromalacia patellae who met the inclusion criteria and did not meet the exclusion criteria were included. All the the patients were from outpatient of Gonarthrosis Dept.in Luoyang Orthopedic Hospital of Henan Province(Orthopedic Hospital of Henan Province),from February 2017 to December 2017.After signing the informed consent form, the patients were divided into observation group(33 persons)and control group(33 persons)randomly according to the order of outpatient. The patients in the control group were instructed to perform straight leg /load-bearing leg raising, squat against the wall statically, and stretch out the knee by seating / stretch out the knee by load-bearing exercises. In the observation group, patients were instructed to perform the training of raising one leg straightly with lateral and lying position, clipping the ball with facing up and lying position on the basis of the exercise treatment program in the control group. Before treatment, the baseline data and clinical symptoms of the two groups of patients were counted, and the WOMAC osteoarthritis index,Kujala scores, and the thigh circumference of 15 cm upper the patella were collected. When the trial started, the patients were trained according to the corresponding functional exercise programs and followed-up by communicating with telephone timely to perform the corresponding exercise treatment guidance. The above criteria were evaluated again one month and three months after the patients had undergone exercise therapy according to the test requirements. After three months,a comprehensive efficacy was evaluated combining with the clinical symptoms of the patients.Results1)The WOMAC pain scores presented a time-dependent decreasing trend in both of the 2 groups, while the 2 groups were inconsistent with each other in the decreasing trend of WOMAC pain scores. There was no statistical difference in WOMAC pain scores between the 2 groups before treatment and at 1 month after treatment(P>0.05).There was statistical difference in WOMAC pain scores between the 2 groups at 3 months after treatment(P<0.05).2)The WOMAC function scores presented a time-dependent decreasing trend in both of the 2 groups, while the 2 groups were inconsistent with each other in the decreasing trend of WOMAC function scores. There was no statistical difference in WOMAC pain scores between the 2 groups before treatment(P>0.05).There was statistical difference in WOMAC function scores between the 2 groups at 1 month and 3 months after treatment(P<0.05).3)The Kujala scores presented a time-dependent increasing trend in both of the 2 groups, while the 2 groups were inconsistent with each other in the increasing trend of Kujala scores. There was no statistical difference in Kujala scores between the 2 groups before treatment(P>0.05).There was statistical difference in Kujala scores between the 2 groups at 1 month and 3 months after treatment(P<0.05).4)The thigh circumference(15 cm upper the patella)presented a time-dependent increasing trend in both of the 2 groups, while the 2 groups were inconsistent with each other in the increasing trend of thigh circumference. There was no statistical difference in thigh circumference between the 2 groups before treatment and at 1 month and 3 months after treatment(P>0.05).5)The total effective rate of the observation group was 93.33%,while the control group was 80.65%.The difference was statistically significant(P<0.05).ConclusionOral medications therapy combined with exercise therapy can effectively improve the clinical symptoms of chondromalacia patellae moreover, exercise therapy for the intensive exercise of the adductor muscles can achieve better results.1)Exercise therapy for the intensive exercise of the adductor muscles can relieve the symptoms of pain before the knee, but the effect of this relief is slow.2)Exercise therapy for the intensive exercise of the adductor muscles can effectively improve the knee joint function of patients with chondromalacia patellae.3)Through the intervention of exercise therapy, the muscle capacity of patients with chondromalacia patellae may be slightly increased, but the intensive exercise of the adductor muscles group does not bring about an increase in muscle capacity that is better than that of the control group.
