Poverty caused by illness is one of the most important reasons for the formation of poor people in China.The accessibility of medical service for the rural poor has become a hot issue of concern to the whole society.Based one the above background,this papaer selected Enshi Tujia and Miao Autonomous Perfecture(hereinafter referred to as Enshi Perfecture)of Hubei Province as the reasearch object through theoretical analysis and empirical research.Through the analysis of literature,field research,and mathematical and statistical analysis,we will study the impact of basic medical insurance on the use of medical service for poor households.The findings are as follows:First,the research area has formed a variety of medical security systems based on the new rural cooperative medical insurance system supplemented by medical insurance,medical assistance,and family health care at the institutional level.Second,through the survey of 1,096 peasant households in 8 villages in 4 counties and cities in Enshi Prefecture in 2016,the variance analysis method was used to compare the utilization of medical services among 814 non-impoverished households and 282 poor households.The study found that compared with non-impoverished households,poor households have poorer physical fitness,lower willingness to seek treatment,heavier medical burdens,and lower awareness of medical insurance policies,but they have a higher rate of reimbursement for medical expenses and higher medical insurance participation ratio.Thirdly,the study of six typical cases found that in the prioritization of family member medical service utilization,juvenile patients are given priority over adult patients,patients with major illness are given priority over patients with common diseases,and the main labor force is given priority over non-workforce patients.Women in the child-bearing period have priority over other members.The fourth is through Logistic model analysis of 282 poverty-stricken households' medical service utilization.The study found that:(i)The similarities and differences between years of poverty will not have a significant impact on whether or not poor households visit,but whether the poor households are willing to be hospitalized or are Hospitalization in the county has a great influence.(ii)Participation in medical insurance will increase the inpatient intention of county hospitals in poverty households,but it will have no significant impact on the intention to attend a visit and to be hospitalized.The willingness of Poor households to attend a visit,to stay in hospital,and choice of hospitalization location will be affected by the evaluation of medical insurance.The proportion of reimbursement has no effect on the hospital visit,hospitalization,or hospitalization at the county hospital of the poor households.(iii)The factors affecting the sick farmers' visits,hospitalizations,and hospital admissions to county hospitals also include disease types and hospital convenience.Based on this,this paper puts forward some countermeasures and suggestions to improve the rural medical insurance system in Enshi,Hubei,and improve the use of medical services for poor households.
