Burn and scalds are one of the most common injuries in daily life,especially infants and young children,and become high-risk groups,because of their lack of self-protection awareness.The light damage the skin of patients,the severe will endinger the lives of patients.Though,modern cosmetic surgery can be carried out on some patients recovery treatment,the patient must under the high financial costs and go through tremendous psychological pressure during the process.Against this background,there is an urgent need to reduce the pain caused by burn and scalds,accelerate the healing of wounds and directly reduce the formation of scar area.The subject came from private and confidential prescriptions,which had the effects of heat-clearing,detoxifying and anti-infection.It could reduce the formation of scar tissue and promote the formation of new skin tissue,and had a good clinical efficacy,but its related efficacy and mechanism had not been reported.Therefore,this research intends to conduct a systematic study for the project.Objective:According to the function of the scald cream,we had a deeply study of the safety,anti-inflammatory,analgesic,antibacterial effect and its mechanism of promoting wound healing to reduce scar formation to ensure its safety,effectiveness and reasonable use in a wide range of the scald cream,what's more,lays a solid foundation for expanding the use of drugs and further development.Methods:(1)The safety test: First,the rat back skin irritation.By knowledge had been found,after the direct application of drugs,observed the changes of the skin redness,swollen,behavioral,mental status and diet,and had a knowledge of compound scald cream topical skin irritation changes.Second,the biochemical indicators of liver and kidney function detection: By testing the administration of 28 days before and after rat liver and kidney function changes in the index to predict the danger of long-term use of drugs on the human body.(2)Antibacterial experiments.The antibacterial effect of the scald cream was observed by a paper method to screen common bacterial species that caused the infection.(3)The main pharmacodynamic experiment.First,the anti-inflammatory test: To observe the ear swelling and toe swelling experiment;Second,the analgesic experiment: By hot plate pain method to observe the efficacy of compound burns cream analgesic effect: Third,through the establishment of mouse blade model to observe the compound scald cream on wound healing and reduce the effect of scar;Finaly,the establishment of scalded rat model to observe the inflammation,infection,wound recovery comprehendsive performance,through the wound healing area directly intuitive display of its healing.(4)Compound scald ointment to promote wound repair and reduce scarring mechanism.First,we could observe the skin dermis,epidermis and appendages of the skin repair to explore the promotion of new skin by HE staining;Second,we could detected the expression of Collagen?in scalded tissues by immunohistochemistry.Thirdly,we could have an evaluatation by ELISA assay of serum TNF-? and VEGF content.Lastly,we could have a study the effect of compound scald paste on promyelocytic tissue of the protein expression of PI3 K / AKT / m TOR was determined by Western blotting.Results:(1)The safety evaluation: the scald cream on the back of the rat skin did not produce redness and other conditions,the large No skin irritation on the mouse surface;no impact on their behavioral activities,mental state,diet,liver and kidney function tests before and after comparison,no abnormalities,no toxic manifestations.(2)Antibacterial experiment: Escherichia coli and Pseudomonas aeruginosa which was Inoculed in the Petri was not inhibition zone formed.However,the Staphylococcus aureus infiltrating compound scald ointment paper produced a size of(6.22 ± 0.13)mm inhibition zone.(3)The pharmacodynamic experiments: First,toe swelling test: Compared with the blank group,carrageenan injection half an hour later,the first 3 to 5 hours,the swelling degree of the toe was significantly different between the administration group and the blank group(P<0.01).Second,ear swelling experiment: after the inflammation induced by xylene,the increase(0.0023 ± 0.0012g)in the treatment group(0.0030 ± 0.0021)g was significantly less than that in the blank group(0.0030 ± 0.0021g)(P <0.01).Third,the hot plate test: the pain threshold was increased in the treatment group(24.0 ± 11.6)S and the blank group pain threshold increased(8.8 ± 3.8)S,there is a significant(P <0.05)increase effect;fifth,the mouse wound test: after the wounds formated,the fifth day after treatment,compound burns cream group that animals tend to have wound healing,the tenth days,the dosing group had all had the tendency of reducing the wound and healing of the incision.On the 16 th day,the scars disappeared,the skin was completely covered by the hair and the wound healed.Finaly,the rat scald experiment: on the fourteenth day,compared with blank group area(0.5 757 ± 0.0916g),the wound area of the treated group(0.7864 ± 0.0996g)had a significant decrease(P <0.01),and there was no infection in the administration group.(4)The mechanism of the compound scald ointment in promoting wound healing and reducing scar formation,as follows: First,compared with the blank group,the administration group results of HE staining showed that the time of the dermal layer differentiating into the epidermis was shorter;Second,the results of Immunohistochemistry about collagen I synthesized in the tissue gap was faster than the administration group.Third,the content of TNF-? in the treated group(52.77 ± 6.53)pg/m L was significantly higher than that in the blank group(222.14 ± 3.74pg/m L)(P <0.01).On the 14 th day,the content of VEGF in the treatment group(116.08 ± 1.07)pg/m L was significantly higher than that in the blank group(108.19 ± 2.69)pg/m L(P <0.01),and there was also a significant difference(P <0.05)compared with the positive group(111.59 ± 0.26)pg/m L.Lastly,in the Westernblot experiment,compared with the control group,the content of PI3 K,AKT,m TOR was increased,and there was a significant difference(P <0.05).Conclusion:The scald cream had a certain anti-inflammatory,analgesic,bacteriostasis,wound healing,and scar repair effects on deep II degree burns caused by the outside world.It mainly reducd the release of inflammatory cytokines TNF-? and regulated the PI3K/AKT/m TOR signaling pathway to promote the synthesis of Collagen I and VEGF,accelerate the differentiation of the dermis to the epidermis,and promote the repair of damaged tissues to complete the remodeling of the skin.
