Objective: To investigate the safety,feasibility and clinical efficacy of laparoscopic surgery in patients with transverse colon cancer.Methods: A retrospective analysis of clinical records of 97 consecutive patients undergoing radical operation of colon cancer between March 2002 and November 2016 was performed by the colorectal surgeons affiliated to Union Hospital,Fujian Medical University.All cases were obtained R0 resection and pathological diagnosis,divided into laparoscopic group of 61 cases and the traditional open group of 36 cases,evaluation of two groups of patients with surgical indicators,postoperative recovery indicators,postoperative complications and survival differences.Kaplan-Meier method was used to analyze the survival of patients between laparoscopic group and traditional open group.The 5-year disease-free survival rate and 5-year overall survival rate were compared by Log-rank method.Results: 1.General information,baseline data such as sex,age,BMI,ASA score,tumor location,tumor size,gross tumor type,degree of differentiation,history of previous abdominal surgery,and underlying diseases among the laparoscopic group and the traditional laparotomy group show no significant difference(P> 0.05).2.intraoperative and postoperative recovery,compared with conventional laparotomy,laparoscopic transcranial cancer surgery patients with less bleeding [(88.4 ± 68.6)ml vs(131.7 ± 79.8)ml,P = 0.009],surgery The duration of postoperative anal exhaust was earlier([(2.9 ± 1.1)d vs(3.4 ± 1.2)d,P = 0.048],and the time to prolongation(226.4 ± 64.2 min vs 194.0 ± 48.9 min,P = 0.011)The postoperative anal defecation time was shorter([3.5 ± 1.2] d vs(4.4 ± 1.3)d,P = 0.001].Other indexes included the number of lymph nodes(28.5 ± 13.1)vs(30.1 ± 18.1),P = 0.604],incidence of postoperative complications [30.1%(19/61)vs 22.2%(8/36)],days postoperative hospital stay [(9.3 ± 5.2)d vs(12.6 ± 10.7)days,P = 0.099 ] had no statistical significance(P > 0.05).The group of 97 cases of median follow-up time of 33(8-91)months.The 5-year overall survival rates of laparoscopic and conventional laparotomy groups were 88% and 84.7% respectively,and the five-year disease-free survival rates were 81.1% and 79% respectively,with no significant difference between the two groups(P > 0.05).Conclusions: Laparoscopic surgery is safe and reliable for the treatment of transverse colon cancer.Compared with traditional surgery,laparoscopic-assisted radical resection of laparoscopic surgerys have less bleeding and postoperative recovery of intestinal function is faster.Not only obtaining similar oncology results,laparoscopic surgery can also achieve favourable long-term prognosis compared with traditional laparotomy.
