Objective: Serum pepsinogen is the biomarker of gastric mucosa,it reflects the gastric atrophy parts and can be a biomarker for gastric screening.The diagnostic value of serum pepsinogen is a question to resolve at presents,so our project is to study the diagnostic value of serum pepsinogen in gastric screening in order to provide theoretical basis for the use of serum pepsinogen in gastric screening.Methods:(1)Select early or advanced gastric patients randomly from January 2016 to December 2016 who was diagnosed by gastroscopy and biopsy.The fasting venous blood was extracted under the admission of the subject.Serum level of PG I and PG II was evaluated and the diagnostic value of PG I,PG II and PG I/PG II was calculated.(2)The serum level of PG I and PG II in gastric cancer high risk group patients from January 2016 to December 2016 was tested,the one with PGI?75ug/L and PG I/PG II?3.0 was supposed to be gastric cancer high risk people,and they were subjected to gastroscopy and biopsy to test for gastric cancer.Results:(1)We had evaluated the serum level of PG I and PG II in 58 gastric patients.The value of PG I,PG II and PG I/PG II in early stage gastric cancer patients was 68.2525.44 ng/ml,24.158.77 ng/ml and 3.451.55.The value of PG I,PG II and PG I/PG II in advanced gastric cancer patients was 61.2128.84 ng/ml,21.589.36 ng/ml and 3.251.98.PGI?75ug/L and PG I/PG II?3.0 was supposed to be the diagnostic standard of gastric cancer and the specificity,sensitivity and AUS was 63.5%,61.2% and 0.725.(2)We had select the gastric cancer high risk person in given conditions and 452 person were selected and their serum PG I and PG II were evaluated,which were 95.6827.59,15.366.77 and 6.952.55.Divided these people into subgroups by sex,age,smoking status,drinking status and familial gastric cancer history,we had found no difference of serum PG I and PG II in those groups.PGI?75ug/L and PG I/PG II?3.0 was supposed to be cut off value of gastric cancer high risk people and 35 of 452 person were positive,the positive rate was 7.74%.21 of them had been examined for gastric cancer but no one was diagnosed with it,this may be the number of enrolled subjects was too small,so more persons should be enrolled in this projects.(3)The value of PG I,PG II and PG I/PG II in early stage gastric cancer patients was 68.2525.44 ng/ml,24.158.77 ng/ml and 3.451.55.The value of PG I,PG II and PG I/PG II in advanced gastric cancer patients was 61.2128.84 ng/ml,21.589.36 ng/ml and 3.251.98.Serum PG I and PG II of gastric cancer high risk group was 95.6827.59,15.366.77 and 6.952.55.According to the data,we could draw the conclusion that the serum level PG I,PG II and PG I/PG II of healthy controls were significantly differ from the gastric cancer patients and serum PG can be a biomarker for gastric cancer screening.Conclusion: Serum PG can be used in early gastric cancer screen and the cut off value PGI?75ug/L and PG I/PG II?3.0 have a high accuracy for gastric cancer high risk people screen.
