Objective The normal position of pelvic organs is closely related to the biological level of pelvic floor support group.Collagen is the main component of extracellular matrix protein in pelvic floor,and micro RNA-29a/b and transforming growth factor bata1 are closely related to collagen metabolism.The purpose of this study was to investigate the changes of expressions of micro RNA-29a/b,collagen,?and transforming growth factor-? 1 in vaginal anterior wall tissues of the patients with prolapse of pelvic floor organs(POP),and to explore the possible mechanism of the pathogenesis of POP.Methods 1.subjects:With the approval of the ethics committee of our hospital and the informed consent of the patients,40 patients with benign lesion hysterectomy were collected from January 2016 to December 2016 in our hospital.According to POP-Q quantitative examination,20 patients were divided into prolapse group(n = 20)and control group(n = 20).The prolapse group was treated with pelvic reconstruction surgery after hysterectomy,who were patients with POP stage III and stage IV.While,the control group was a group of patients whose POP-Q classification for hysterectomy due to uterine leiomyoma or adnexal benign mass was less than stage I.No diagnosis of chronic pelvic inflammatory disease,endometriosis,congenital connective tissue disease,malignant tumor existed in these patients.2.Research technique: The expression level of miR-29 was detected by real-time fluorescence quantitative polymerase chain reaction(q RT-PCR),and the expression of type I,III collagens and transforming growth factor-? 1 were detected by Western blot.3.Statistical analysis was conducted with SPSS 20.0 Software.Results 1.Comparison of miR-29a/b expression among the groups: the expression of miR-29a/b was significantly higher in the group with POP than that in the group without POP(7.36 vs.1),and the difference was statistically significant(P<0.001).2.The expression of Collagen III was significantly lower in POP group than that in the control group(0.73±0.17 vs.1.11±0.27,P=0.00);and the expression of Collagen I in POP group was lower than that in control group,but there was no significant difference between the two groups(0.78±0.79 vs.1.32±1.76,P=0.116).The expression of TGF-? 1 in prolapse group was significantly higher than that in the control group(2.72±3.12 vs.1.73±2.33,P=0.020).3.Pearson bivariate correlation analysis: there was a negative correlation between the decrease of type I collagen protein and the increase of miR-29a/b expression(r=-0.664,P=0.036)in the anterior wall of vagina in prolapse group,and a positive correlation between the decrease of type III collagen protein and the increase of miR-29a/b expression(r=0.637,P=0.048).There was a positive correlation between the increase expression of TGF-?1 and the increase of miR-29a/b expression(r=0.660,P=0.040).But there was no significant correlation between the expression of miR-29a/b and type I,III collagen and TGF-?1 in the control group(P > 0.05).Conclusions 1.The decrease of collagen type III in the tissue of pelvic floor may be involved in the pathogenesis of POP.The increase of TGF-?1 plays a role in the pathogenesis of POP.The increased expression of miR-29a/b in the anterior wall of vagina in POP patients suggests that epigenetics may also be involved in the pathogenesis and development of POP.2.The decrease of type I and III collagen and the increase of TGF-?1 were significantly correlated with the increase of miR-29a/b expression in the anterior wall of POP,suggesting that the decrease of collagen content and the increase of TGF-?1 content in the anterior wall of vagina of POP patients may be due to the increase of miR-29 expression.
