| ObjectiveTo study the relationship between cytomegalovirus(CMV),herpes simplex virus(HSV),and EBV(EBV)infection and the disease activity of systemic lupus erythematosus(SLE)and ankylosing spondylitis(AS).Objects and methodsThis study is divided into two parts.To investigate the relationship between viral infection and SLE,clinical data and laboratory data of 28 patients with SLE and 38 healthy women were collected continuously.Viral DNA of IgG,IgM,oral epithelial cells,and peripheral blood of CMV and HSV in serum was detected.,CMV,HSV);To investigate the relationship between virus infection and AS,continuous collection of clinical data and laboratory data of 123 AS and 38 healthy men,and detection of serum CMV,HSV IgG,IgM and oral epithelial cells,peripheral Blood,virgin synovial fluid,and synovial virus DNA(EBV,CMV,HSV).Results 1 The relationship between CMV infection and SLE.The CMV-IgG positive rate of SLE was not significantly different from that of the control group,but the CMV-IgG titer was higher than that of the control group.Both CMV-IgG positive and CMV-IgG titers were not associated with disease activity in SLE.The positive rate of CMV-IgM was significantly higher in the SLE group than in the control group(35.71% VS 0),and the SLEDAI score was higher in the CMV-IgM positive group(Z=-2.005,P=0.045),and the complement C3 was lower(Fisher,P).= 0.03),but complement C4,ESR,CRP and negative were not significantly different.In the SLE group,only 1 case of CMV-DNA was positive,the corresponding disease activity index was higher,and the CMV-DNA of the control group was negative.If CMV-DNA or CMV-IgM positive is defined as CMV active infection,the SLEDAI score of CMV active infection is higher than that of no active infection(22(17)VS 11(9.5),P=0.024),complement C3 was lower than those without active infection(P<0.05),but there was no significant difference in complement C4,ESR,and CRP.2 The relationship between HSV infection and SLE.The positive rate of HSV-IgG and the titer of HSV-IgG in SLE were not significantly different from those in the control group,and were not significantly related to the disease activity index.The positive rate of HSV-IgM in SLE was significantly higher than that in the control group(48.14% VS10%),and the SLEDAI score was significantly higher in HSV-IgM positive than in the negative(20(16.75)VS 11(8.25),P=0.039),but complement There was no significant difference between C3,C4,ESR,and CRP.In the SLE group,only HSV-1 DNA was detected in 3 oral epithelial cells,and the corresponding disease activity index was higher.No HSV-1 DNA was detected in the peripheral blood and the control group.No HSV-2 DNA was detected in both SLE and control groups.If the HSV-IgM or HSV-DNA positive is defined as active HSV infection,the SLEDAI score in patients with active HSV infection in the SLE group is significantly higher than those without active infection(18(16)VS 10(8),P=0.028),but there was no significant difference in complement C3,C4,ESR,and CRP.3 The relationship between EBV infection and SLE.The EBV-DNA positive rate of SLE in oral epithelial cells and blood was not significantly different from that of the control group,and there was no correlation between EBV-DNA positive and disease activity index of SLE.If EBV-DNA positivity in the blood is defined as EBV active infection,EBV active infection is not related to the disease activity index of SLE.4 The relationship between CMV infection and AS.The positive rate of CMV-IgG in AS was not significantly different from that of the control group,and the positive of CMV-IgG was not associated with the disease activity index of AS.The CMV-IgG titer of the AS group was lower than that of the control group,but the higher the CMV-IgG titer,the higher the BASDAI score(R=0.581,p=0.001)and the longer the medical history.There was no significant correlation with the ESR and CRP.Both the AS and control groups were negative for CMV-IgM.The positive rate of CMV-DNA in AS was not different from that in the control group.The oral specimens were 8.8% and 8.3%,respectively.The blood and synovial fluid and synovium were negative.Among them,the disease activity indicators corresponding to CMV-DNA positive were higher.5 The relationship between HSV infection and AS.The HSV-IgG positive rate and HSV-IgG titer in the AS group were not significantly different from those in the control group,and there was no significant relationship with AS disease activity.If 18 is used as the dividing line between high and low HSV-IgG titers,the ESR of HSV-IgG high titer is significantly higher than that of low titer(27.50±27.33 VS 21.97±13.84,P=0.001),but there is no significant difference between CRP.difference.The HSV-IgM positive rate of AS was not significantly different from the control group(8.94% VS 10%),but the corresponding ESR of HSV-IgM positive was significantly higher than that of negative,and the difference of CRP was not statistically significant.In the AS group,HSV-1 DNA was detected only in 4 oral epithelial cells,and the corresponding disease activity index was higher.HSV-1 DNA was not detected in the peripheral blood,synovial fluid or synovial membrane,and the control group.HSV-2 DNA was not detected in both AS and control groups.6 The relationship between EBV infection and AS.The EBV-DNA positive rate of AS in oral epithelial cells,blood,synovial fluid or synovium was not significantly different from that of the control group,and there was no correlation between EBV-DNA positive and AS disease activity index.Active EBV infection is not associated with AS disease activity indicators.Conclusion 1 Active CMV infection(CMV-IgM or CMV-DNA positive)is associated with disease activity in SLE.Active HSV infection(HSV-IgM or HSV-DNA positive)is associated with SLE disease activity.EBV infection has nothing to do with the disease activity of SLE.2 The higher the CMV-IgG titer,the higher the BASDAI score for AS.The higher the HSV-IgG titer,the higher the ESR of AS,and the higher ESR of HSV-IgMpositive patients than negative ones.EBV infection has nothing to do with the disease activity of AS. |
