Objective Study on the relationship between exercise and exercise-related behavior and blood pressure control in hypertensive patients and influencing factors.In order to provide scientific basis for people to choose proper and effective intervention methods,and to provide reference for optimizing the exercise intervention program for hypertension patients in China.Methods 1 Meta analysis of effect of exercise intervention on patients with hypertension A meta-analysis of randomized controlled trials of exercise interventions published in 2012 and 2016 published in China and abroad on Chinese patients with hypertension was conducted through Chinese and English databases such as CNKI.Measured data are described using the weighted mean difference(WMD)and its 95% CI.At the same time,sensitivity analysis wasperformed.Egger's test were used to evaluate whetherthere was publication bias.2 Effects of exercise and exercise related behaviors on blood pressure control of hypertensive patients Taking a hypertensive patient in an urban community in Fuzhou as the research object,a non-matching case-control study was conducted with the effect of blood pressure control as the standard.Questionnaire survey of patients using systematic sampling.SPSS17.0 software was used for statistical analysis.Count data using?2 test.Unconditional logistic regression model for factors affecting hypertension control by univariate and multivariate analysis.3 Qualitative research Using literature research and semi-structured interviewing methods to understand the current status,difficulties,and causes of exercise management in hypertensive patients in the community,and to summarize related issues and suggestions.Results 1 Meta analysis of effect of exercise intervention on patients with hypertension(1)The meta-analysis showed that after exercise intervention,the exercise intervention group reduced systolic blood pressure(WMD=-10.71,95%CI:-14.56~-6.86),diastolic blood pressure(WMD=-5.77,95%CI:-8.26~-3.28),Total cholesterol(TC)(WMD =-0.50,95%CI:-0.79~-0.20),triglyceride(TG)(WMD=-0.38,95%CI:-0.59~-0.16),low-den sity lipoprotein cholesterol(LDL-C)(WMD=-0.86,95%CI:-1.37~-0.36),BMI(WMD =-0.98,95%CI:-1.89~0.07),and elevated high cholesterol(HDL-C)(WMD=-0.45,95%CI:-1.10~-0.13)compared with the control group,the differences were statistically significant(P<0.05).(2)Subgroup analysis showed that the intervention group had statistically significant blood pressure control compared to the control group(P<0.05).(3)Meta analysis results are stable and reliable.2 Effects of exercise and exercise related behaviors on blood pressure control of hypertensive patients(1)Multivariate logistic analysis showed that the factors that affect blood pressure control in hypertensive patients are:exercise OR=0.194,regular household chores OR=0.397,intermittent monitoring of blood pressure OR=2.412(P<0.05).(2)Multivariate logistic analysis of exercise-related behavioral control of blood pressure showed that walking OR=0.189 and exercise time(60 to 120 minutes per day)OR=0.31 correlated with the effect of blood pressure control(P<0.05).(3)Gender(Female)OR=2.248,Sports are accompanied by family OR=0.048,Health belief model OR=0.016 are the factors that influence exercise compliance(P<0.05).3 Qualitative research The status of exercise management activities in hypertensive patients in the community:It is conducted in the form of traditional health education;the general practitioner team is the main body of education;the community health promotion manual is relatively shallow;patients have low coordination and lack of motivation.Conclusion Exercise intervention has a positive effect on improving blood pressure,blood lipids,and BMI in hypertensive patients in China.There is a correlation between exercise-related behavior and blood pressure control in middle-aged and elderly patients with hypertension.In the current community,hypertensive patients have low enthusiasm for physical activity.Measures should be taken at different levels to promote patient movement.
