Objective: The study of clinical efficacy and safety of glaucoma in the treatment of trabeculectomy with Ologen.Methods: Collected patients who was diagnosed with primary glaucoma after a series of perfect checks from September 2015-November 2016 treatment in Chengdu University of Traditional Chinese Medicine Hospital,A total of twenty-six cases,twenty-six eyes.Those patients were randomly divided into experimental group and control group.The experimental group was treated with trabeculectomy with Ologen and the control group received a simple trabeculectomy.After the operation,the follow-up time was 6 months.The patients were followed up at 1,7,14,30,60,90 and 180 days after operation.Need to record the preoperative basic information and basic examination of patients and the situation of Postoperative follow-up about best corrected visual acuity(BCVA),intraocular pressure,the type of bleb,Shallow anterior chamber incidence,concomitant medication,postoperative complications and adverse reactions and 180 days of computer dynamic field of view,nerve fiber layer thickness(RNFL),subfoveal choroidal thickness(MFCT).The results were analyzed by SPSS21.0 statistical software.The measurement data were expressed by mean ±standard deviation.The classification data were analyzed by chi-square test.Group comparison and comparison between groups before and after treatment with the normal distribution using t test,non-compliance with non-parametric test.Results:1.Baseline dataComparison of the two groups preoperative data and eye examination,there were no significant differences,comparable.2.Analysis of postoperative indicators2.1 IOPThe IOP of the two groups at 1,7,14,30,60,90,180 days after operation was significantly lower than that of the preoperative and the difference was statistically significant(P <0.05).There was no significant difference between the two groups about the postoperative IOP.There was no significant difference in the success rate between the two groups,In the experimental group,the amplitude of IOP reduction was higher than that of the control group within 2 months after operation,and the amplitude of iop was stable compared with the control group.2.2 BCVACompared with preoperative,two groups of BCVA during the seven follow –up,The BCVA of the 180 th day of the trial group was statistically significant compared with preoperative,The best corrected visual acuity of the control group on day 90 was compared with preoperative with significant statistical differences.There was no significant difference between the two groups in the best corrected visual acuity of seven times follow-up after surgery.2.3 filter bubbleThere was no statistically significant difference between the two groups at the last follow-up about the type of filter bubble.But the experimental group of 13 cases were all functional filtration bleb,the control group appeared 1 case of functional filter bleb.2.4 MS and MDThe mean light sensitivity and mean defect after surgery were compared between the two groups,P> 0.05,there was no significant difference.Comparison with preoperative and postoperative mean light sensitivity and mean defect,There was no significant difference in the test group and the control group.But from the mean point of view,The average postoperative sensitivity was higher than that before operation.The mean defect was smaller than that before operation.2.5 SFCTThe subfoveal choroidal thickness after surgery was compared between the test group and the control group,P> 0.05,no significant statistical significance.The differences of preoperative and postoperative SFCT was statistically significant in both groups,The SFCT of the control group and the experimental group was thicker than that before the operation.26 cases of patients according to TCM syndrome differentiation,analysis of different syndromes of patients with SFCT,the results showed that the patients SFCT with syndrome of hyperactivity of yang due to yin deficiency thinner than the patients with syndrome of phlegm-fire disturbing mind and heat syndrome due to stagnated liver qi,the difference was statistically significant.2.6 RNFLCompared with the thickness of each quadrant of the nerve fiber layer in the control group and the control group,p> 0.05,there was no significant difference.In the experimental group,postoperative RNFLcompared with preoperative,In addition to the quadrant of TI,the difference was statistically significant.The RNFL in the control group was statistically significant compared with that before operation,in addition to the quadrant of NI.The RNFLin the two groups was lower than that before the operation.But the degree of thinning of the test group was smaller than that of the control group.2.7 Postoperative complications and adverse reactionsIn the control group,there were 4 cases of I degree shallow anterior chamber in the early postoperative period,In the experimental group,there were 1 case of intraocular pressure increased with ? degree shallow anterior chamber patients.The shallow anterior chamber formation rate in the control group was significantly higher than that in the experimental group.There were no other complications and adverse reactions in both groups.Conclusion:1?It is effective about the application of Ologen in trabeculectomy for glaucoma treatment.2?It is safe about the application of Ologen in trabeculectomy for glaucoma treatment.3?After glaucoma surgery,in the case of iop successfully reduced,we still need to pay attention to the protection of optic nerve treatment.4?After trabeculectomy,the patient's SFCT thickened;The SFCT of patients with yin deficiency and yang hyperactivity syndrome was significantly lower than that of patients with syndrome of phlegm-fire disturbing mind and heat syndrome due to stagnated liver qi.The subfoveal choroidal thickness in glaucoma patients with different syndromes may be different.
