Objective : To evaluate the efficacy and safety of SLIT with Dermatophagoides farinae drops in children with asthma and allergic rhinitisMethods:1、The object of study:between 2012 and 2017,72 children with asthma and allergic rhinitis who accepted Dermatophagoides farinae drops were recruited form the second hospital of Tianjin medical university,age 4-to 15-year old.64 children with asthma and allergic rhinitis who did not accepted Dermatophagoides farinae drops in the same period as control group,age 4-to 15-year old.According to the treatment duration,children were divided into 3 groups(1-year,2-year,3-year).According to the serum allergen(s Ig E),children were divided into dust mite allergy group(Dermatophagoides farinae or Dermatophagoides farinae and Dermatophagoides pteronyssinus sensitization)and polysensitized group(dust mite and other allergens multiplex sensitization)2、Observation indicators:asthma symptom score(day and night),rhinitis symptom score,total symptom score,medication score,VAS(visual analog scale)score of asthma and rhinitis,pulmonary function testing(percentage of FEV1),Fe NO.Results:1.At baseline there were no significant differences in asthma symptom score(ASS),rhinitis symptom score(RSS),total symptom score(TSS),medication score(MS),VAS score of asthma and rhinitis,FEV1 and Fe NO between different courses of SLIT groups(P>0.05).2.There were no significant differences in ASS,RSS,TSS,MS,VAS score of asthma and rhinitis,FEV1 and Fe NO between different courses of Control groups at baseline(P>0.05).3.There were no significant differences in ASS,RSS,TSS,MS,VAS score of asthma and rhinitis,FEV1 and Fe NO between different courses of SLIT and Control groups at baseline(P>0.05).4.Compared with baseline,the ASS,RSS,TSS,MS,VAS score of asthma and rhinitis were significantly decreased in different courses of SLIT groups(P < 0.05).No significant differences were found among each SLIT groups(P > 0.05).The percentage of predicted FEV1 were significantly increased after different courses of SLIT,the differences were statistically significant(P<0.05).There was no significant difference in Fe NO levels before and after treatment in each groups.MS in 3-year group and 2-year group was significantly lower than 1-year group(P<0.05).There were no significant differences in MS between 3-year group and 2-year group.5.Compared with baseline,the ASS,RSS,TSS,MS,VAS scores of asthma and rhinitis were significantly decreased in different courses of Control groups,the differences were statistically significant(P<0.05).The percentage of predicted FEV1 and Fe NO were not significantly different from those before treatment(P>0.05).There were no significant differences in the ASS,RSS,TSS,MS,VAS scores of asthma and rhinitis,FEV1 and Fe NO among each groups(P>0.05).6.There were significant differences in ASS,RSS,TSS,MS,VAS scores of asthma and rhinitis among different courses of three SLIT groups and three Control groups after treatment,the differences were statistically significant(P < 0.01).There were no significant differences in FEV1 and Fe NO levels compared with the control groups(P>0.05).7.After SLIT,the TSS,the TSS in the dust mite allergy group and the polysensitized group were significantly lower than baseline,the differences was statistically significant(P < 0.05).In the control group,TSS were significantly improved after treatment in children with dust mite allergy group and polysensitized group.There were no significant differences in TSS between dust mite allergy group and polysensitized allergic group after SLIT(P=0.26).In the control group,there were no significant differences in TSS between dust mite allergic group and polysensitized group after treatment(P=0.19).8.No serious adverse reactions were observed in each SLIT and control groups.One patient in the 2-year treatment group complained of drowsiness after vaccination and was soon relieved after one week.Conclusion:1.This study found that both different courses of SLIT and the control groups can improve the clinical symptoms of children.2.SLIT with Dermatophagoides farinae drops in children with allergic rhinitis and asthma was security and had better clinical efficacy than that of the control groups.3.SLIT with Dermatophagoides farinae drops in children with allergic rhinitis and asthma for more than 2 years has better clinical efficacy.
