Objective To observe and evaluate the efficacy and safety of 2-years SLIT of DFD for children with allergic asthma.Methods 110 cases aged 6-14 years with non acute exacerbation of mild-moderate allergic asthma from outpatient clinic of asthmatic diseases of Fuzhou Children’s Hospital were divided randomly into experimental and control group, each of 55 cases, from March to June in 2012. The experimental group was treated with sublingual DFD and conventional treatment in accordance with the GINA, and the control group was treated with conventional treatment only. The security and the scores of DNAS,C-ACT were evaluated each month. The s Ig G4, s Ig E, skin prick test were evaluated every 6 months and the FEV1%, FEF25%, FEF50% and PEF of pulmonary function test were detected every 12 months after treatment. The number of respiratory tract infections and asthma attacks, the children missed school days, and IC dose were recorded through the 2-years follow-up.Results1.The FEV1%, FEF25% and FEF50% of pulmonary function, the value of s Ig G4 and the score of C-ACT in experimental group was higher than control group for 24 months treatment( t=-2.146 P=0.005, t=-2.185P=0.031, t=-3.098 P=0.003, t=-7.583 P=0.0001, t=-2.146 P=0.034). And the score of DNAS was lower than the control group(t=2.183 P=0.035, t=2.103 P=0.03), and the difference was statistically significant.2.The dust mite skin test level changes, s Ig E and the PEF of pulmonary function for 24 months treatment in the experimental group showed no differences with the control group(χ2=0.510 P=0.185, t=-0.183 P=0.855, t=-1.877 P=0.063).3.The FEV1%, FEF25% and FEF50% of pulmonary function after treatment were higher than before treatment in experimental group(t=-5.973 P=0.0001, t=-2.431 P=0.019, t=-3.957 P=0.0001), and the difference was statistically significant.4. The FEV1% of pulmonary function after treatment were higher than before treatment in control group(t=-6.201 P=0.0001), the FEF25% and FEF50% of pulmonary function showed no differences between afterand before treatment in control group(t=1.461 P=0.150, t=-1.770 P=0.083).5.The value of s Ig G4 in experimental group increased year by year between treatment for 12 months and 24 months, the difference of s Ig G4 between the experimental group and the control group is statistically significant(F=44.722 P=0.0001).6. The number of respiratory tract infections and asthma attacks, the children missed school days in experimental group significantly reduced than the control group after treatment( t=-2.409 P=0.020, t=-3.194P=0.002, t=-2.304 P=0.026), and the difference was statistically significant.7. The IC dose in experimental group significantly reduced than the control group after treatment(χ2=9.072 P=0.028).Conclusion1. SLIT of DFD could improve the pulmonary function of children with allergic asthma and the scores of children asthma control test and daytime, nighttime asthma symptom.2. SLIT of DFD could reduce the number of respiratory tract infections and asthma attacks, the children missed school days and IC dose.3. SLIT of DFD could produce s Ig G4 and the s Ig G4 value was increased significantly and relieved the symptoms of allergic reactions year by year in the specific immunotherapy of sublingual dust mite for a two-years therapy.4. DFD could be a safe and effective treatment of allergic asthma in children-specific immunosuppressive drugs.
