Investigation Of Club Drug Use And Infection Of HIV Among Men Who Have Sex With Men In Tianjin

Obeject:To learn about the use of club drugs,high risk sexual behavior and HIV infection status for the men who have sex with men in Tianjin,analysis of the influencing factors of HIV infection in the population.And then it can provide information for the comprehensive prevention and control of AIDS,i t also provides a scientific basis for the more effective formulation of health education and intervention measures for high risk population in AIDS.Methods:To use the cross section investigation method,from April to August in 2015 and from August to October in 2016,with the assistance of the MSM community group,use the non-probability sampling method,recruit 328 qualified respondents by rolling snowball(330 in all,removal of 2 unqualified).Using a face-to-face way to conduct a questionnaire survey,content includes general demographic data,the use of the six most common new drugs,sexual behaviour in the last three months,and cognition of the risk of self HIV infection.After completing the questionnaire survey,the serological detection of HIV in the venous blood was collected for each of the subjects,confirmatory test for positive sieves.All survey data were recorded in Epi Data3.0,statistical description and analysis by using SPSS 22.0.Statistics were investigated for the use of club drug insertions,infection of HIV,and awareness of the status of the infection of the MSM.The χ2 test was used to compare the count data between groups,analysis of the influence factors of HIV infection by multi factor unconditional logistic regression,“whether or not the infection of HIV” is the dependent variable,other variables are covariate.Results: A total of 328 MSM were investigated,the average is 29.74±9.485,the youngest is 17 and the oldest is 70.Club drug users account for 54%.The single use of one and the mixed use of two or more accounted for 72.3% and 27.7% of the drug addicts respectively.The highest proportion of RushPoPPers was used,and the time for the first use of club drugs was mainly concentrated in 2011-2015.Among different populations,under the age of 40 years(69.2%、57.4% vs 27.9%),the cultural level in high school and above(52.0%、67.3% vs38.4%),the average monthly income of more than 5000 yuan(75.0% vs 58.5%、33.9%),non Han ethnic(88.9% vs 51.9%),gay and bisexual orientation(67.2%、38.1% vs 0.0%)using more club drugs than others,the proportion of the average monthly income of more than 5000 yuan mix than other income groups,the difference was statistically significant(P<0.01,P<0.05).During the last three months,The frequency of intercalative behavior of new drug users once or more times a week,the number of sexual partners and 2,and the proportion of participatory group behavior is higher than non users(36.2% vs 16.6%、23.7% vs 9.3%、4.5% vs 0.0%,P<0.01).The proportion of unprotected sex with male sex workers and other temporary partners is higher than that of non users(14.7%vs6.6%,P<0.05;36.2%vs13.9%,P<0.01)The proportion of new drug users who had never used condoms was higher than that of 155 other temporary partners is higher than non users(24.8%vs6.5%,P<0.01).There are 223 people thought they might be infected with HIV,accounting for 68%,and the others believed that it is impossible to infect or not know.,accounting for 32%.In the actual test results,there were 25 people with positive HIV,and the infection rate was 7.6%.The actual infection status of people who thought they were not likely to be infected with HIV or unknown was "confirmed positive ",the proportion of them was higher than that of others(18.1% vs 2.7%,P<0.01).Through non conditional logistic regression analysis,The variables that affect HIV infection are unprotected with other temporary partners and club drugs use of syringes to use with others.The OR of unprotected with other temporary partners is 15.657,95% CI is 4.750~51.610,shows that a risk factor for HIV infection by expressing unprotected sex with other temporary partners,the risk of infection is 15.657 times that of other people.The OR of syringes to use with others is 40.350,95% CI is 2.069~786.924,shows that sharing syringes with others to use new drugs is a risk factor for HIV infection,,the risk of infection is 40.350 times that of other people.Conclusions:The proportion of new drugs used by the MSM population in Tianjin is higher than that of other cities,the club drug using the highest proportion is Rush PoPPers.The ratio of new drug users to high-risk sexual behavior is significantly higher than that of other people,The risk factors for HIV infection in the MSM population are unprotected sex with temporary partners and sharing syringes with others.The long-term use of club drugs will indirectly increase the risk of HIV infection.MSM people are less cognition of the risk of self HIV infection.Therefore,it is necessary to strengthen the effective control of club drugs,strengthening the emphasis on propaganda and intervention for high risk population at the same time.
