Purpose:To observe and analyze the eccentricity of the optic zone after wearing a night-worn orthokeratology lens,to investigate the influence of different decentralized conditions on the visual quality,and to analyze the high-order and full-corneal aberrations and different light conditions before and after wearing the lens.Contrast sensitivity changes.Compare and analyze high-order aberrations after wearing orthokeratology or guided wavefront guided femtosecond laser in situ keratomileusis.Methods:(1)Retrospective analysis of the clinical data of patients who were treated with overnight orthokeratology lenses.The corneal topographic maps were examined at 1 week,1 month,and 3 months after wearing glasses.In the post-mirror corneal topography,the amount of eccentricity(including the eccentricity distance and angle)of the center of the orthokera flattening area with respect to the pupil center was measured,and the degree of eccentricity,the degree of astigmatism,and the equivalence between the eccentricity center and the patient's wearing glasses were analyzed.The relationship between spherical and flat corneal curvature.Eccentric distances and angles were compared at each time point after wearing a mirror with a one-way analysis of variance,and correlation analysis was performed using person or pearson correlation analysis.(2)Using the Sirius 3D Anterior Segment Analysis System to detect corneal aberrations,WaveScan Wavefront AberrationTester to detect wavefront aberrations,and using the aberration results at4 mm and 6 mm of the cornea and pupil diameter to analyze the anterior and posterior aberrations of wearing glasses.In the case of changes,the area under the curve of the contrast sensitivity function of the contrast sensitivity and its calculation was used to analyze the change in sensitivity before and after wearing the glasses.Compare the difference in contrast sensitivity and aberrations under different off-center conditions.(3)Patients with refractive surgery guided by wavefront shaping lenses and those with wavefront aberration were selected.Patients with similar spherical aberration,degree of astigmatism,and corneal curvature were compared and analyzed.The aberrations of the whole eye and the cornea after treatment were compared.Result:1.The average eccentricity at the center of the optical zone at 1 week,1month,and 3 months after wearing glasses was(0.65±0.41)mm,(0.59±0.39)mm,and(0.55±0.26)mm,respectively,at various time points after wearing glasses.The eccentricity distance is mainly light and medium eccentricity,and there is little severe eccentricity.The eccentric direction is generally dominated by the temporal malleolus.The eccentricity of the temporal axis is mostly in the horizontal direction,and the eccentricity is the majority in the vertical direction.There is no significant difference in the eccentric distance and direction at each time point after wearing the microscope(P>0.05).2.The correlation analysis showed that the eccentricity distance at 1m and 3m was correlated with the initial spherical aberration(r=0.222,p=0.048;r=0.337,p=0.04),and the eccentric distance of 1w was related to the initial astigmatism.(r=0.398,p=0.018),the eccentricity distance isnot related to the equivalent spherical lens power and flat corneal curvature.3.After 3 months of glasses,HOA,Sph,Coma,Trefoil,Z-1 3,and Z1 3under 4mm corneal diameter were all significantly increased(P<0.05).AOA and wearing glasses were significant.Compared with the decrease,the difference was statistically significant(P<0.05).In the total ocular aberration of 4mm pupil diameter,HOA,Sph,Coma,Z-1 3,and Z1 3were significantly increased compared with those before wearing glasses.The difference was statistically significant(P<0.05).Before AOA and wearing glasses,Compared with the decrease,the difference was statistically significant(P<0.05),and the Trefoil difference was not statistically significant(P>0.5).4.In the study of contrast sensitivity before and after wearing glasses,under high light conditions,the CSF in the high frequency region(18c/d)increased after wearing glasses(1.21±0.177)than before wearing glasses(1.15±0.205).Under low-light conditions,the CSF in the low frequency region(3c/d)decreased after wearing glasses(1.68±0.182)compared with wearing glasses(1.75±0.166),and the difference was statistically significant(P<0.05).There was no significant difference in other results(P>0.5).5.According to the eccentric distance,the patients wearing orthokeratology lenses were divided into group A(eccentric distance<0.5mm)and group B(eccentric distance> 0.5mm).In corneal aberration,the aberration values ??of group A under 4mm cornea diameter were all lower than that of group B(P<0.05);the total aberration,spherical aberration and clover difference of group A under 6mm cornea diameterwas lower than that of group B.In group B,there was no significant difference in other aberrations(P>0.05).In total ocular aberration,the spherical aberration in group A(0.11±0.04)?m was lower than that in group B(0.13±0.05)?m in 4 mm pupil diameter,and the difference was statistically significant;the spherical aberration in group A was less than 0.5 mm in 6 mm pupil diameter.A group was lower than B group,and the difference was statistically significant.There was no significant difference in other aberrations(P>0.05).In the comparison of contrast sensitivity,the contrast sensitivity of low frequency(3c/d)and high frequency(18c/d)in group B was lower than that in group A,and the difference was statistically significant.The difference in the other contrast sensitivity did not show statistical significance.Comparing the AULCSF under each light condition,the AULCSF in the dark light glare in the B group was lower than that in the A group,and there was no statistically significant difference in the AULCSF between light and dark light..6.After 3 months of treatment in FS-LASIK and wearing glasses group,HOA,Sph,Coma,Trefoil,Z-1 3,and Z1 3 in the wearer's group under4 mm corneal diameter were all high in corneal aberration.In the surgery group,the difference was statistically significant(P<0.05).Under the6 mm cornea diameter,HOA,Sph,Coma,and Z-1 3 in the wearer group were higher than those in the surgery group,and the difference was statistically significant.Z1 3,Trefoil difference No statistical significance(P>0.05).In total eye aberration,HOA,Sph,Coma,Trefoil,and Z1 3 in the wearer's group with 4mm pupil diameter were all higher than those in the surgery group,and the difference was statistically significant(P<0.05).There was no statistical difference in Z-1 3.Significant(P>0.05),6mm corneal diameter,wearing glasses group HOA,Sph,Coma,Trefoil,Z-1 3higher than the surgery group,the difference was statistically significant,Z1 3 difference was not statistically significant(P> 0.05).Conclusions:1.The center of the optical zone can occur during orthokeratology wearing,most of which is mild to moderate,and maintains a stable state after wearing for one week.The occurrence of eccentricity is related to the pre-optic state of refraction and the physiological shape of the cornea..2.After the wearing of the microscope,the cornea and the higher-order aberrations of the whole eye have increased in different degrees,but there is no significant effect on the contrast sensitivity;the centrality of the wear affects the higher-order aberrations of the cornea and the whole eye,and the greater the eccentricity,the higher order The more the aberration increases,the greater the eccentricity is in the contrast sensitivity of dark and dim glare.3.Comparison of FS-LASIK and Orthokeratology for total higher-order aberrations and corneal higher-order aberrations in the treatment of myopia,wearer's eye and corneal HOA,Sph,coma,Trifoil,Z-1 3,Z1 3All were higher than the surgery group,and the difference in spherical aberration was the most obvious.This was related to the smoother transition between corneal and peripheral corneas in the central optic zone of FS-LASIK after treatment than keratoplasty.The cornea was more wearable after FS-LASIK treatment.After shaping the lens has a better visual quality.Both treatment methods have their own advantages,and the treatment methods should be selected according to the specific clinical conditions.
