| Objective: Chronic obstructive parotitis is not rare among the patients with Sj?gren’s syndrome,but it is seldom reported in the literature.The traditional ways of treatment,like massaging or drugs to stimulate saliva production,irrigating catheter with help of medicine(iodized oil,antibiotics)and parotidectomy,haven’t receive satisfactory curative effect.The aim of the study is to evaluate the effectiveness of sialendoscope for chronic obstructive parotitis in patients with Sj?gren’s syndrome,whereby providing reference to clinic use.Design: 22 patients with chronic obstructive parotitis related to Sj?gren’s syndrome were selected for study from May 2016 to July 2017 at the Department of Oral and Maxillofacial Surgery,School of Stomatology,China Medical University(Shenyang,China).The patients received routine salivary ultrasonography,salivary ECT and sialography examination before the operation.Two methods are used to evaluate the sialendoscope curative effect for chronic obstructive parotitis in patients with Sj?gren’s syndrome.First,compare patients’ the uptake and secretion rate of the affected parotid gland before and after the operation.Paired t-test was adopted in statistical analysis and P value was statistically significant(P<0.05).If the patients’ the uptake and secretion rates of the affected parotid gland rise evidently six months later after the operation,it will be proved that the parents’ parotid gland function of up-taking and secretion is improved,and sialendoscope treatment for chronic obstructive parotitis in patients with Sj?gren’s syndrome is effective.Second,compare the visual analogue scale(VAS)before and after the operation within six months.According to the paired t-test,the results of P value(P < 0.05)were considered to be statistically significant.If VAS drops evidently six months later after the operation,then patients’ parotid symptoms are alleviated and sialendoscope treatment works.Results: The 22 patients(37 parotid glands were successfully received interventional treatment through VAS under local anesthesia).The parotid glands ECT preoperative average uptake ratio is 0.92±0.51‰,dropped to 0.90±0.50‰ six months later after the operation,which is not significantly in statistic analysis(P>0.05).The preoperative average secretion rate is 42.90 ± 15.36%,significantly increased to 47.24 ± 17.13%(P<0.05)six months later after the operation.The preoperative average score of VAS was5.95±0.79,and the average score of VAS decreased to 4.45±1.44(P < 0.05)six months after the operation.Conclusions: The study indicates the interventional sialendoscope treatment improves patients’ chronic obstructive parotitis,decrease the frequency of parotid swelling,relieve dry-mouth symptom and significantly increase parotid gland function of excretion(parotid gland function of uptaking varies insignificantly).In conclusion,the interventional sialendoscope plays a vital role in treatment of chronic obstructive parotitis related to Sj?gren’s syndrome. |
