?Objective?To analyze the features of CBC T image in the diagnosis of submandibular gland stones and chronic obstructive parotitis.To compare C BCT with traditional imaging techniques such as bite film and ultrasound.To explore the advantages and disadvantages of the imaging techniques above.3D imaging technique of simplant software is applied for 3D imaging of stones and parotid duct.To explore the morphological and obstructive causes of parotid duct and provide guidance for clinical treatment.?Methods?1.Patients in the ninth people's hospital salivary gland disease specialist clinic of Shanghai Jiao Tong University,from August 2015 to April 2016,with the clinical history of pain or swelling in the submandibular gland were enrolled in the research.Medical history,clinical examination,ultraso und,bite film and C BCT were examined.Simplant software was used for 3D imaging of CBC T imaging to measure the stones' size.The stone area is parted according to the teeth(1-3 mandibular teeth for the front,4-7 for the middle and later part including 8 and glands).According to the result of operation,explore the accuracy of all kinds of different methods.2.Patients in the ninth people's hospital salivary gland disease specialist clinic of Shanghai Jiao Tong University,from August 2015 to April 2016,with the clinical history of pain in the parotid gland region or repeatedly gland swelling related to feeding were enrolled in the research.Medical history,clinical examination,conventional ultrasound examination and CBCT sialography were examined.3D imaging technique of Simplant software was used to reconstruct the parotid duct,and the morphological characteristics of parotid duct and the related causes of obstruction were analyzed.?Results?1.The incidence rates of calculus in anterior,middle and posterior region in 110 patients were respectively 20.9%,39.1%,40.0%.The detection rates of bimanual examination in anterior,middle and posterior calculus were respectively 91.3%,74.4%,11.4%.The detection rates of bite film in anterior,middle and posterior calculus were respectively 78.2%,83.7%,11.4%.The detection rates of ultrasound in anterior,middle and posterior calculus were respectively 56.5%,81.4%,86.4%.The detection rates of CBC T in the anterior,middle and posterior calculus were respectively 100%,96.8% and 96.9%.The results showed that there were some differences in the detection rate of stones in different regions using different examination methods.2.14 patients with 18 sides were taken radiography CBC T diagnosed chronic obstructive parotitis(COP).The rates of detection in the different branch ducts were 100%,94.4%,88.9%,66.7%,respectively.3D imaging showed that the lesions of the parotid duct were irregular stenosis and dilation,and the lesions were located in the middle section of the parotid duct.?Conclusions?(1)there was no significant difference in the incidence of calculus in submandibula gland in different gender,age groups(P>0.05).(2)the detection rate of calculus in the posterior region with bite film was low,the detection rate of ultrasound for the anterior calculus was lo w.the overall detection rate of CBC T was high,and had statistical difference comparing with bite film and ultrasound(P<0.05)(3)CBCT sialography represent the main,first and second branches,parts of third and fourth branches of the parotid gland ducts and can clearly locate the stenosis.(4)CBCT can be the effectively supplement for the clinical diagnosis of obstructive sialadenitis,especially for the diagnosis of small stones and and localization of parotid duct stenosis and provide reliable evidence for clinical treatment.
