Objective To Compare The Clinical Effect Of Shock Wave Combined With Diacerein Capsules For The Treatment Of Knee Osteoarthritis

Objective: Through retrospective analysis of 90 outpatient cases,a comparative study was compared with the clinical effects of extracorporeal shock wave and Diacerein Capsules on knee osteoarthritis before,during(in the treatment of1 month,2months)and after treatment(1 month after the end of treatment).Method: By collecting patients admitted from the Affiliated Hospital of Chengdu University of Traditional Chinese Medicine from June 2016 to November 2017 and diagnosed with knee osteoarthritis according to the clinical diagnostic criteria,90 patients were screened according to the inclusion criteria and exclusion criteria.According to the different treatment methods,90 cases were divided into three groups,including shock wave therapy group(30 cases),Diacerein Capsules group(30cases)and extracorporeal shock wave combined with Diacerein Capsules group(30cases).The three groups were recorded before,during(in the treatment of1 month,2months)and after treatment(1 month after the end of treatment).VAS pain soring,WOMAC scoring and WOMAC pain index scoring were conducted in all patients before,during(in the treatment of 1month,2months)and after treatment(1month after the end of treatment).In addition,before and after treatment,the knee joint color ultrasound examination was performed to compare the changes in the effusion of the knee joint before and after the treatment,and then the results were collected and analyzed statistically.The patient's adverse reactions and other discomfort during the whole treatment were recorded.Results:1.There was no significant difference among the three groups in average age,gender,BMI and K-L grade(P>0.05),having the comparability of the experiment.2.There was no significant difference among the three groups of patients in VAS pain scoring,WOMAC scoring and joint effusion volume(P> 0.05),which had experim-ental comparability.3.During the treatment of one month,Extracorporeal shock wave combined with Diacerein Capsules group was significantly lower than the other two groups in VASpain scoring,WOMAC scoring,P<0.05,and the difference was statistically.The shock wave group decreased significantly than diacerein capsules group,P<0.05,which was statistically significant.During the treatment of two months and after treatment(1 month after the end of treatment),there was no statistical significance between the shock wave group and the Diacerein Capsules group,P>0.05.4.the amount of joint effusion among the three groups after treatment decreased significantly.there was statistically significant compared with the three groups,P<0.05.5.After treatment,only 4 patients had no obvious therapeutic effect,the total efficient rate of shock wave group amount to 90.0%,supass the Diacerein Capsules group86.7%,with 5 patients invald.the total effective rate of the combined treatment group was the most,only 1 cases were invalid and the total effective rate amount to 96.7%,indicating that the effect of the combined treatment group was the most obvious.Conclusion: 1.Both shock wave and diacerein capsules were effective at lowering VAS scoring and WOMAC scoring in patients with KOA.The improvement of symptoms with arthralgia is obvious,and the combination of shock wave and diacerein capsules is more effective in the treatment of knee osteoarthritis.2.During the treatment period of one month,shock wave and diacerein capsules,used to treat patients with KOA,could reduce the pain and other related clinical symptoms,but the shock wave had a faster onset;during the treatment period of the second month,the two groups had comparable treatment effect.3.At the end of the course of treatment,comparisons within and between groups showed that all three groups could effectively improve the clinical symptoms of the patients,indicating that the three groups had longer-lasting sustained effects,in which the difference in efficacy of various index between shock wave group and the diacerein capsules group was not significant,while the combined group had obvious advantages and improved clinical symptoms more significantly.4.The joint effusion of the combined treatment group decreased significantly after treatment,and the best effect was obtained.The shock wave group was superior to thediacerein capsules group.5.In the three groups during treatment,shock wave group,shock wave combined with diacerein capsule group showed respectively 4 cases and 3 cases of skin ecchymosis at the joints,accompanied by numbness,no special treatment,and disappeared on their own.Three patients in the Diacerein capsule group experienced mild diarrhea discomfort after taking the drug,no other adverse reactions,and disappeared with no treatment.Most of the patients,taking diacerein capsules,experienced yellowish urine symptoms,and were informed that it was a normal metabolic reaction of the drug and disappeared after discontinuation.6.Compared with diacerein capsules,shock waves have the advantages of convenient operation,short treatment time,fewer adverse reactions,quicker onset,and better effect than diacerein alone.Therefore,the use of diacerein capsules can be combined with shock wave therapy to achieve better clinical efficacy.
