| This paper take document analysis of hospital efficiency evaluation at home and abroad as the main method,deeply investigates the situation of comprehensive reform of public hospitals in Zhongxian County,see the progress of the reform policy,and analyzes the basic situation and operation of the pilot hospitals.In order to reflect the impact of the reform on county-level public hospitals,rethink the difficulties in the reform process to find solutions,and to learn lessons from the experience,this paper uses the data envelopment method(DEA)to analyzes the operational efficiency of the pilot hospital andprovide reference for promotion of the reform and policy improvement.This research has found that in the pilot hospitals,the policy has achieved certain effects since the pilot hospitals implemented comprehensive reform of county-level public hospitals.However,some policies have not been implemented,and the price of medical services has not been adjusted;At present,hospitals do not havethe autonomy to employ personnel,and haven't implement a system of competitive selection and appointment for a dean;the remote diagnosis network has not yet been established;in terms of the operation of pilot hospitals,the input of resources has increased compared with previous years,and the supply of medical services has increased year by year.The balance of income and expenditure has decreased to a certain extent,in which medical business income,basic financial subsidy income and drug income have all shown rising signs,and cost expenditure has also shown the opposite,and the increase rate has exceeded income;in addition,From the point of view of hospital operation efficiency,compared with things before the comprehensive reform,has improved.But the change is not significant,the static efficiency,the overall efficiency of Chinese medicine hospital is higher than that of the people's hospital,but the dynamic efficiency,The total factor productivity of the people's hospital only showed positive growth in 2014-2015 after the reform,and at other time there was all negative growth;the total factor productivity of the Chinese medicine hospital was all negative growth,but after the reform,its negative growth trend was slowed down.Through qualitative and quantitative analysis,it is not difficult to see that both the people's Hospital of pilot County and the Hospital of traditional Chinese Medicine have different degrees of growth in basic input,but the total amount of human resources is still relatively low,and the balance of income and expenditure of thepilot hospital becomes smaller.The public welfare of public hospitals has been strengthened,however,the structure of income and expenditure still needs to be improved;the charges for sub-average drugs are reduced,the pressure on patients to use drugs is weakened,but the overall medical burden is still relatively heavy.In addition,after the comprehensive reform,the operation efficiency of the pilot hospital fluctuated up and down,but it did not show a long time.In view of the above problems,we should constantly improve the government investment mechanism to ensure the implementation of public welfare,and improve the personnel compensation system,improve the quality of human resources,adjust the price of medical services,optimize the income structure,further strengthen the fine management of hospitals,Improve the operational efficiency in order to fully demonstrate the effectiveness of comprehensive reform. |
