Purpose:By observing and comparing the visual quality of postoperative patients with 1.8mm c-mics and 3.2mmc-sics,mainly including visual acuity,astigmatism of cornea and sugery induced astigmatsim,intraocular pressure,ocular surface changes and corneal edema in postoperative patients.Then,providing theoretical basis and guidance for clinical practice.Methods:Select age-related cataract patients based on inclusion criteria and exclusion criteria who had an operation of Phacoemulsification and intraocular lens implantation in patients in the ophthalmology department of the Affiliated Hospital of Yan'an University during 2016/12~2017/12.The patients were randomly assigned to the experimental group and the control group,then the first 30 eyes were selected according to the time sequence of the patients entering the study.The visual quality of postoperative patients was observed and compared,including postoperative visual acuity,astigmatism of cornea and sugery induced astigmatsim,intraocular pressure,ocular surface changes and corneal edema.The results were collated and analyzed using SPSS22.0 statistical software.Results:1.The preoperative astigmatism of the experimental group and the control group were as follows: 0.80±0.37,0.86±0.45,no statistically significant difference after T Test(t=0.50,p=0.62);after comparing two groups of corneal astigmatism,the two groups of astigmatism for one week duration were: 0.99±0.36,1.38±0.51,the difference was statistically significant(t=3.30,p=0.002),for two months duration,the two groups of astigmatism were: 0.87±0.34,1.09±0.40,the difference was statistically significant(t=2.30,p=0.026).At one week after operation,the two groups of SIA were: 0.69±0.19,1.07 ± 0.24,the difference was statistically significant(t=6.71,p=0.000),after two months,the two groups SIA were: 0.58±0.17,0.88±0.25,respectively,the difference is statistically significant(t=5.40,p=0.000).Two months after surgery,the two groups of corneal astigmatism were larger than the preoperative astigmatism,and the difference was statistically different(t=4.51,p=0.00;t=4.89,p=0.00),while the 3.2mm group was more significant.2.For the experimental group and the control group,the preoperative uncorrective distant visual acuity LogMAR were: 0.82±0.34,0.93±0.37,respectively,the difference was not statistically significant(t=1.11,p=0.27);the best corrective vision acuity LogMAR before operation were: 0.52 ± 0.52,0.52 ± 0.44,there was no statistically significant difference(z=0.23,p=0.82).The Uncorrective distant visual acuity LogMAR first day after the operation were: 0.22 ± 0.19,0.30 ± 0.30,the difference was statistically significant(z=2.02,p=0.04),one week after the operation,the ncorrective distant visual acuity vision LogMAR were: 0.15±0.12,0.22±0.23,there were statistically significant differences between the two groups(z=2.31,p=0.02),and two months after the surgery,the two groups of Uncorrective distant visual acuity LogMAR were: 0.10±0.12,0.13±0.12,no statistically significant difference(z=0.52,p=0.61).The corrected vision acuity LogMAR of the two groups at one week after operation were: 0.10±0.11,0.10±0.22,no statistically significant difference(z=0.48,p=0.63)was observed.3.The preoperative intraocular pressure(mmHg)of the experimental group and the control group were: 14.16±2.35,14.43±2.54,the difference was not statistically significant(t=0.42,p=0.68);one day after operation,the two groups of intraocular pressure were: 15.67 ± 3.0,16.10 ± 3.14,the difference was not statistically significant(t=0.55,p=0.59);one week after surgery,the two groups of intraocular pressure were: 14.90±2.31,15.01±2.35,there was no statistically significant difference(t=0.17,p=0.87);two months after surgery,the following two groups of intraocular pressure were: 13.83±2.29,14.07±2.33,the difference has no statistical difference(t=0.39,p=0.67).There were significant differences in intraocular pressure between preoperative and postoperative for both 1.8mm group(t=2.14,p=0.04)and 3.2mm group(t=2.54,p=0.02)one day after surgery.There was no statistically significant difference between the two groups one week after surgery of intraocular pressure(t=1.51,p=0.14 and t=0.65,p=0.26),and there was no statistically significant difference between the two groups two months after surgery(t=0.96,p=0.34 and t=1.01,p=0.32).4.1 The preoperative BUT(seconds)of experimental group and control group were: 9.53±3.39,9.63±2.63,the difference was not statistically significant(t=0.15,p=0.88).Two days after operation,two groups of BUT were: 6.73±2.16,6.47±2.19,there was no statistically significant difference(t=0.47,p=0.64),the BUT of the two groups for one week after surgery were: 8.13±2.06,7.07±2.03,and the difference was statistically significant(t=2.03,p= 0.048);the two groups of BUT two months after operation were: 9.07 ± 2.15,8.67 ± 1.97,no statistically significant difference(t=0.75,p=0.46).For two months after surgery,there was no statistically significant difference of BUT for 1.8mm group(t=1.85,p=0.08),but was statistically significant for 3.2mm group(t=4.00,p=0.000)compared to preoperative difference.4.2 There was no statistically significant difference between the experimental group and control patients in DES before the operation(z=0.48,p=0.63);the difference was not statistically significant(z=0.83,p=0.41)at two days after operation,while the difference was statistically significant(z=2.12,p=0.034)one week after surgery,i.e,1.8mm group was less than 3.2mm group in terms of DES;two months after operation,there was no statistically significant difference in DES between the two groups(z=1.45,p=0.15).4.3 The preoperative SIT(mm)of 4.3 and control group were: 11.53±3.19,11.13±2.43,no statistically significant difference(t=0.55,p=0.59).Two days after the operation,the SIT for the two groups were: 17.93±4.82,20.70±4.81,the difference was statistically significant(t=2.23,p=0.03).One week after the operation,SIT were: 14.20±3.94,15.03±3.59,respectively,no statistically significant difference(t=0.86,p=0.40).Two months after the operation,SIT were: 11.13±3.07,12.13±3.91,the difference has no statistically significant(t=1.20,p=0.24),and there was no statistically significant difference for both1.8mm and 3.2mm groups compared to preoperative sit(t=0.88,p=0.39 and t=2.0,p=0.06).5.There was no statistically significant difference of corneal edema between the two groups either one day or one week after the surgery(z=0.26,p=0.79 and z=0.57,p=0.57).Conclusion:1.The corneal astigmatism,the source astigmatism and the short-term vision 1.8mm group were superior to the 3.2mm group in two kinds of incision cataract after phacoemulsification.2.After two kinds of incision cataract phacoemulsification,the 1.8mm group had less effect on the stability of the tear film,and the comfort was better than that of the 3.2mm group.3.The effect of phacoemulsification on the degree of intraocular pressure and corneal edema in two kinds of incision cataract is basically the same.
