The Application And Resource Barriers Of Case Management In Serious Diseases Relief

The current medical assistance system is still not perfect in our country,most of the social disadvantaged groups are still free from the relief scope,especially serious medical treatment there is still a very big problem,because many patients cannot afford the high medical costs and lead to can not get proper treatment.Such groups often face multiple and complex demands,and their social support is weak,and it is difficult for them to access social resources to solve their own problems.Therefore,medical social workers need to take the case management method to intervene,through teamwork,linking and integrating internal and external resources to provide services.The research question of this article is to explore the medical institutions in case management is how to apply in the rescue service of a serious illness,including the application status of case management in the rescue of a serious illness and dilemmas.This article attempts from the perspective of resource dependence theory,using qualitative case-study methods,the families with severe and severe diseases and the medical social workers who carried out the case management in G hospital in Guangzhou as the main research object,based on the G hospital in Guangzhou social work services in participatory observation,through the application for the rescue of a serious illness in-depth interviews to understand the object facing the difficulties and requirements to meet the status quo,through the analysis of case management process in the application of G hospital rescue of a serious illness,digging out the case management present situation of the application in the rescue of a serious illness,aswell as difficulties and limitations.Then we analyze the causes of case management from the perspective of resource dependence theory,and further summarize the feasibility Suggestions and strategies for developing case management in the practice of medical rescue service.By analyzing and summarizing,case management as a work method of integrating social work in harmony,to face various difficulties of a serious illness patient and family to provide efficient and perfect service,it is to help the service object the establishment of comprehensive network of resources to help them get a certain ability to solve problems has unique advantages,the service mode to provide a reference for the researchers,practitioners and other relief area.
