| ObjectiveBased on home oxygen therapy and oral administration of acetylcysteine effervescent tablets,bu Fei yi Shen Tang combined with methylprednisolone tablets were used to treat interstitial lung disease(ILD)caused by lung and kidney qi deficiency.the clinical efficacy of the treatment was compared with that of methylprednisolone tablets alone.the clinical symptoms,signs improvement,lung function results,arterial oxygen partial pressure,chest high resolution computed tomography(HRCT)performance,6-minute walking test and quality of life were observed.the adverse reactions were also recorded.the efficacy and safety of the treatment were also observed.MethodsSixty-eight patients with lung-kidney qi deficiency ILD from April 2016 to August 2017 in respiratory and internal medicine clinics of Shanxi province traditional Chinese medicine hospital were randomly divided into treatment group and control group according to random,controlled and equal proportion.34 patients in each group were treated with oral methylprednisolone tablets combined with bufeiyishen decoction on the basis of home oxygen therapy and oral acetylcysteine effervescent tablets.the control group was treated with oral methylprednisolone tablets alone for 12 weeks.The improvement of western medicine comprehensive clinical efficacy score,Chinese medicine clinical symptom score,lung function,arterial blood oxygen partial pressure,chest HRCT,6-minute walking test and quality of life in and between the two groups before and after treatment were observed.statistical software SPSS 20.0 was used to carry out data statistics and evaluate the clinical efficacy of the two groups.ResultsThe sex,age,course of disease,severity of illness and disease distribution ratio of two groups of ILD patients with lung-kidney qi deficiency before intervention were statistically analyzed by statistical software.the results were not statistically significant(P > 0.05),and statistical comparison could be made.After 12 weeks of treatment,the research results are as follows:1.in western medicine comprehensive clinical curative effect improvement: afterthe treatment group and the control group were given different clinical intervention measures,the comprehensive clinical curative effect scores of the two groups were significantly lower than before treatment(P < 0.05).after treatment,the scores of the treatment group were significantly lower than that of the control group(P < 0.05).2.in the improvement of TCM symptom score:(1)after different clinical intervention measures were given to the treatment group and the control group,the scores of short breath after treatment in the two groups were significantly lower than before treatment,indicating that the symptoms of short breath were relieved,the difference was statistically significant(P < 0.05),the scores of the treatment group were significantly lower than that of the control group,the statistical difference was significant(P < 0.05).(2)the scores of cough,expectoration and asthenia in the treatment group were significantly lower than those before treatment(P < 0.05).the scores of the control group were not significantly changed before and after treatment(P > 0.05).3.in terms of pulmonary function improvement:(1)after different clinical intervention measures were given to the treatment group and the control group,the total lung capacity(TLC)data of the two groups before and after treatment did not change significantly(P > 0.05),and there was no significant difference in TLC values between the two groups(P > 0.05).(2)the amount of carbon monoxide diffusion capacity(DLCO)in lung tissue of the treatment group was significantly increased before and after treatment(P < 0.05),while the dlco value of the control group was not significantly changed before and after treatment(P > 0.05).4.in the improvement of pulmonary arterial oxygen partial pressure: after different clinical intervention measures were given to the treatment group and the control group,the value of PaO2 in the treatment group increased significantly before and after treatment,the value of P(A-a)O2 decreased significantly,the statistical difference was significant(P < 0.05),the value of PaO2,P(A-a)O2 in the control group before and after treatment did not change significantly,the statistical difference was not significant(P >0.05).5.in the aspect of chest HRCT imaging improvement: after different clinical intervention measures were given to the treatment group and the control group,there was no significant change in chest HRCT imaging score before and after treatment in the two groups(P > 0.05).6.6 minutes walking distance improvement: the treatment group and the control group were given different clinical intervention measures,the two groups within the treatment after 6 minutes walking distance than before treatment increased significantly(P < 0.05),compared between the two groups after treatment,the treatment group increased the distance significantly greater than the control group,the statistical difference was significant(P < 0.05).7.in terms of quality of life:(1)after different clinical intervention measures were given to the treatment group and the control group,the scores of overall health,physiological function,emotional function and mental health of the patients in both groups were significantly increased before and after treatment(P < 0.05).after treatment,the scores of the treatment group in the above dimensions weresignificantly higher than those in the control group(P < 0.05).(2)the scores of physical pain in the two groups were not significantly increased before and after treatment(P > 0.05).ConclusionBased on home oxygen therapy and oral administration of acetylcysteine effervescent tablets,oral methylprednisolone tablets combined with bufei Yishen decoction can significantly improve the clinical symptoms and signs of patients with interstitial lung diseases,improve their cardiopulmonary endurance and quality of life,improve the dlco index of lung function and arterial oxygen partial pressure,but have no obvious improvement effect on TLC,chest HRCT imaging performance of lung function and body pain. |
