BackgroundBecause the social medical service system in our country is not perfect,most of the patients go to the tertiary hospital regardless of the severity of the illness,which makes the emergency department of the third class hospital overcrowded.When emergency medical resources are inevitably dispersed to non-critical patients,the ability of the entire emergency care system to access critical patients will decline.With the increasing workload of emergency,manual statistics of data and information will be lost to varying degrees,emergency medical data preservation,collection and analysis lack of standardization and efficiency.The lack and lag of emergency information management leads to difficulties in the evaluation,monitoring and analysis of emergency service process,medical quality,workload and content..Medical informatization is the digitization,networking and Informatization of medical services,it refers to the collection,storage,processing,extraction and data exchange of patient information and management information between hospitals and hospital departments through computer science and modern network communication technology and database technology,and meet the functional needs of all authorized users.With the development of wireless information technology,mobile communication technology and Internet of things technology,through the establishment of a pre-hospital integrated information system,it is possible for the clinical data of emergency patients to be effectively preserved and standardized management.The release of “The Guidelines on the Application function of the Hospital Information platform in 2016” and “The Technical Guide on the Application of Hospital Information Construction in 2017” makes the management of medicalinformation become the focus of the new round of medical reform.Strengthening the information construction of emergency department and the clinical application of data has become an urgent problem in the development of emergency department.ObjectiveThrough the application of the integrated information management system of pre-hospital and hospital developed by our hospital and information technology company in emergency department,to analyze the epidemiological characteristics of emergency patients so as to provide guidance for guiding the clinical work in emergency,optimizing the allocation of medical resources and improving the management of departments.MethodFrom January 1,2017 to December 31,2017,this study retrospectively analyzed the basic information and related clinical data of emergency patients collected by the Emergency Information Management system in our hospital(The second affiliated Hospital of Guangzhou Medical University).EXCEL software is used for data collation,chart drawing,and SPSS 23.0(Chicago,IL)statistical software is used for data analysis.All the results were expressed by Mean±SD or Median(IQR),and the measurement data were measured by T test/Rank test.P < 0.05 indicates that the difference is statistically significant.Results1.Pre-hospital emergency patient data show:In 2017,the number of cars in front of the emergency department of our hospital was 5515,the number of patients receiving treatment reached 4813.2371 cases(49.26%)were mild first aid before hospital,1547 cases(32.14%)were moderate,645 cases(13.40%)were severe,250cases(5.19%)were death.8 :00-10:00 was the peak time of the first aid in our hospital,and 17:00-20:00 is the peak time for in-hospital emergency visits.In our hospital,the rate of dispatching vehicles before hospital and meeting the standard of driving time was 99.83%,and the rate of dispatching car to spot time was 99.68%.2.The data of in-hospital emergency patients showed that in 2017,the number of in-hospital visits was 151508,the number of in-hospital patients was 343 cases(0.23%),4895 cases(3.23%)of grade ? patients,88759 cases(58.58%)of grade? patients,and 57511 cases(37.96%)of grade ? patients.The median triage time for different grades of emergency patients in our hospital was 2 minutes for grade ?,4 minutes for grade ?,1 minute for grade ?,and 1 min for grade ?,there was significant difference in the comparison between different triage grades(P <0.05).The median waiting time is grade I 3min,grade II 4 min,grade III 9 min,grade IV 10 min.there were significant differences in the comparison between different triage grades(P < 0.05).3,The first five diseases in the spectrum of pre-hospital emergency were: 1306cases(27.13%)of nervous system emergency,933 cases(19.38%)of trauma,688cases(14.29%)of circulatory system emergency,503 cases(10.45%)of respiratory emergency and 369 cases(7.67%)of digestive system emergency.The first six disease types in the hospital emergency disease spectrum were as follows: 52407 cases(36.86%)of respiratory diseases,28338 cases(19.93%)of digestive system diseases,13630 cases(9.59%)of trauma,10038 cases(7.06%)of nervous system diseases,9177 cases(6.45%)of otolaryngology / oral / ophthalmic diseases,6784 cases(4.77%)of circulatory system.4.The proportion of nervous system emergencies was the highest among the wild,moderate and severe patients in pre-hospital care,35.39%,33.62%,38.65%respectively.The proportion of circulatory system emergency was the highest among the dead patients was 77.31%.The proportion of patients with neurological disease in grade I patients was the highest,accounting for 43.97%,and the proportion of patients in class II was the highest,accounting for 23.98%,and the proportion of respiratory system diseases in grade III patients was the highest,accounting for 37.13%,and the proportion of respiratory system diseases was the highest in IV patients,accounting for 56.30%.5.Age distribution of pre-hospital first aid patients was mainly concentrated in81 ~ 90(19.87%),71~80(17.68%),61~70(14.61%)years old.The age distribution of emergency patients in hospital was mainly divided into three age groups: 0 ~ 14(27.39%),20~ 30(18.66%),31 ~ 40(12.92%)years old.6.The first five points of pre-hospital emergency treatment were as follows: 300 times in Jiangnan Dadao Zhong(6.21%),273 times in Jiangnan West Road(5.67%),261 times in forward road(5.41%),257 times in Xingang West Road(5.33%),300 times in Dongxiao South Road(4.37%).Conclusions1.At present,emergency medical resources cannot be used reasonably.Promoting Emergency Information Management and grading diagnosis and treatmentis an important way to allocate emergency medical resources.2.Emergency of nervous system and circulatory system is the focus of critical management in emergency department,our hospital should pay attention to the Construction of Stroke Center and Chest Pain Center...
