Objective: To study the clinical manifestation and pathological features and clarify the relationship,and emphasize the importance of the patients with IgA nephropathy.Methods: 233 patients who underwent pereutaneous renal biopsy in Department of Nephrology of the first hospital of HuaiHua from 2011.07 to 2017.12,were selected as study subjects,their clinical and pathological informations were analyzed retrospectively.And to analyzed the relationship between clinical and pathological of the patients with IgA nephropathy.Results:In our study,The ratio of male to female morbidity was 1:1.38;the age of onset was 14-73 years old;the average age of onset was36.68±13.41;21-30 years old was the age of high onset;the male and female age distribution was different(P<0.05),with ?50 The difference is most significant in the age segment.2.Clinical information:1)Theincidence of hypertension among the 233 patients was 28.76%(67/233);the serum creatinine was elevated,accounting for 17.17%(40/233);the CKD1 phase was the most frequent,accounting for 51.07%(119/233),and patients with renal failure,0.86%(2/233);abnormal urinalysis was 100%,with hematuria,proteinuria accounted for 49.79%(116/233);proteinuria in a small group,accounting for 45.49%(106/233);large proteinuria group(? 3.5g/24h),accounting for 6.44%(15/233);hyperuricemia accounted for 36.91%(86/233);hypoproteinemia accounted for 8.58%(20/233).2)There was no significant difference in CKD staging and hypertension grading(P>0.05).Protein urine quantification increased with the increase of CKD staging in 24h(P<0.001);hypertensive patients increased with increased uric acid levels.The difference was statistically significant,P<0.05.The number of renal insufficiency increased with the increase of uric acid level,and the difference was statistically significant(P<0.001).3)Pathological data:Focal hyperplasia was the most common pathological type in this study,accounting for 55.79%(130/233);followed by mesangial hyperplasia,accounting for 35.62%(83/233).IgA+IgM type was the most common globulin deposition type,accounting for 87.98%(205/233).The fluorescence intensity was 3+,accounting for 57.51%(134/233).Pure complement C3 deposition was the most common type of complement,accounting for 79.4%(185/233).The M1E0S0T0 was the most common in the Oxford grading,accounting for 55.36%(129/233).The Lee IIIgrading was the highest,accounting for 52.36%(120/233).The differences in age,blood pressure distribution,urinary protein quantification,uric acid distribution,and Lee's grade intensity were statistically significant(P<0.05).With advanced age,increased urinary protein,increased uric acid,and elevated blood pressure,Lee's grade increased.There was no statistically significant difference in the gender distribution in the Lee classification grade intensity(P>0.05).Conclusion:1.The incidence of IgA nephropathy is mainly young and middle-aged,especially in the 20-30 age group;women are more common;there is no significant difference in the average age of men and women;2.The urine tests of IgA nephropathy patients mainly include hematuria and proteinuria,and the 24-hour urinary protein increase,the risk of renal insufficiency increased;3.The most common pathological type of IgA nephropathy is focal hyperplastic changes;4.IgA nephropathy immunopathology was dominated by IgA+IgM combined deposition,IgA fluorescence intensity was mainly 3+,and deposition of complement was mainly C3 deposition;5.In Lee's pathological grade of IgA nephropathy,mainly Lee III grade;the higher the Lee's grade,the higher the blood pressure,CKD high stage,a large number of proteinuria,advanced age,hyperuricemia patientscombined probability of increase;6.The Oxford pathological grade of IgA nephropathy is most commonly M1E0S0T0,and the increase of mesangial cells and intracapillary cells is the most common pathological change.7.Clinical and pathological features of 233 patients with IgA nephropathy in Huaihua area have certain links.
