| Objective:This paper retrospectively analyzed Korean nationality and han nationality in Yanbian clinical and pathological features of primary IgA nephropathy in recent 10 years,and further explore the differences between Korean nationality and han nationality in primary IgA nephropathy,It provides the basis for selecting the most suitable clinical diagnosis and treatment measures.Methods:107 patients with primary IgA nephropathy pathologically diagnosed by renal biopsy were collected and analyzed retrospectively from January 2009 to December 2018 in the department of nephrology of Yanbian hospital,and secondary factors were excluded.Statistical analysis two major categories of Korean nationality and han nationality:(1)general clinical data:the patient’s gender,age,systolic/diastolic blood pressure(mmHg)、simultaneous phenomenon;(2)biochemical index:blood routine,urine sediment quantitative,24 hours urinary protein,total protein,albumin,cholesterol,triglycerides,and estimated glomerular filtration rate(eGFR),serum creatinine and urea nitrogen,the elf inhibition C,C3,immunoglobulin A;(3)pathological files:Immunofluorescence,light microscopy,electron microscopy and other pathological results of renal biopsy were obtained.Results:1.Among the 107 cases with primary IgA nephropathy,the Korean IgA nephropathy patients were older,mean is 41.79,the standard deviation is 12.88;However,the age of nationality IgA nephropathy in Han nationality was younger,mean age is 35.25,the standard deviation is 10.95,the difference was statistically significant;The number of han nationality patients was 51 people,including 25 males(49%)and 26 females(51%).;There were 56 Koreans,28 males(50%)and 28 females(50%),and there was no statistically significant difference in the prevalence rate between males and females.2.Among IgAN patients with concomitant symptoms,the prevalence of hypertension in Korean nationality was 64.3%,while that in Han nationality was 37.3%,Korean nationality was significantly higher than that in Han nationality,with statistical significance;There was no significant difference in hematuria,proteinuria and edema.3.The analysis of the onset time before kidney biopsy showed that both Korean and Han nationalities had the highest percentage of renal biopsy within 3 months,but there was no statistical difference between the two nationalities.4.Comparison of laboratory tests between the Two groups of IgA nephropathy patients of Korean nationality and Han nationality showed that 24-hour urine protein level of IgA nephropathy patients of Han nationality was higher than that of Korean nationality patients,and serum complement C3 level of Korean patients was lower than that of Han group,with statistically significant difference(P<0.05);There was no statistical difference in other biochemical indexes.5.Lee classification and Oxford Classification(MESTC)were used for the pathological changes of IgA nephropathy.The data showed that in the Lee classification,grade Ⅰ accounted for the highest proportion in the Han IgA nephropathy patients(41.2%),while Grade Ⅱ accounted for the highest proportion in the Korean patients(39.3%),and the difference was statistically significant.In Oxford type(MESTC),there was no statistical difference between Han and Korean patients.Conclusion:1.In YanBian area IgAN,the average age of han nationality patients is younger,while the Korean group is older:2.Among IgAN concomitant symptoms,the prevalence of hypertension in Korean patients is higher than that in Han patients;3.Among the assay indicators of IgAN patients,the 24-hour urine protein level of Han patients was higher than that of Korean patients,and the serum complement C3 level of Korean patients was lower than that of Han patients;4.Among the pathological grades of IgAN in Yanbian(Lee’s grade),the han patients had the most grade I pathological changes,while the Korean patients had the most grade Ⅱ pathological changes. |
