Clinical Observation On The Treatment Of Hypoalbuminemia(Deficiency Of Both Qi And Blood)in Elderly Patients After Hip Fracture With Buxue-yushang Granules

Objective:This paper mainly observes the clinical efficacy of Buxue-Yushang Granules combined with oral nutrition(ONS)on the treatment of hypoalbuminemia(deficiency of both qi and blood)in elderly patients after hip fracture surgery and also evaluates the clinical examination indicators,changes in TCM syndromes,adverse reactions of patients and etc.Then it also discussed its mechanism and clinical value in order to conduct a new exploration for the treatment of hypoalbuminemia in elderly patients after hip fracture surgery.Methods:Through exclusion and screening,70 patients who met the inclusion criteria were chosen as the experimental objectives.They were randomly divided into treatment group and control group,of whom 35 patients were in treatment group and35 patients were in control group.All cases were operated by the same group of physicians,and the anesthesia method was continuous epidural anesthesia.Intertrochanteric fracture of the femur in the orthopaedic surgical bed under the traction of the proximal femur anti-rotation intramedullary nail(PFNA)surgery,in conjunction with the use of C-arm machine during surgery,perspective position satisfaction,fixed and reliable,fine intraoperative operation,stop bleeding carefully.The control group was given ONS(whole protein enteral nutrition,Ansu)after operation,and the treatment group was given Buxue-Yushang Granules on the basis of drug treatment with the control group.After treatment for 10 days,it was observed and noted following statistics of items,including the clinical symptoms,signs,serum albumin(ALB),total protein(TP),postoperative complications during hospitalization,subjective comprehensive nutritional SGA evaluation,ADL score of out-of-hospital daily living ability and safety detection indicators of the two groups were observed and recorded,and the efficacy and safety of Buxue-Yushang Granules were analyzed and evaluated.Results:(1)Comparison of curative effect of TCM syndromes of diseases: through statistical analysis,the total effective rate of treatment group was 91.43 %,and that of control group was 85.71 %.There was statistical difference between the two groups.The treatment group combined with Buxue-Yushang Granules was superior to the control group in treating hypoproteinemia(deficiency of both qi and blood)after hip fracture in the elderly(P=0.027 < 0.05).(2)Comparison of disease efficacy:through statistical analysis,the total effective rate in the treatment group was 94.29 %,and that in the control group was 91.43 %.There was no statistical difference between the two groups(P= 0.190 > 0.05),and the number of recovery in the treatment group was higher than that in the control group.(3)Laboratory test results: by comparing TP and ALB before and after treatment,there was no statistically significant difference between the two groups before treatment,which was comparable(P >0.05).There was no statistically significant difference between the two groups after treatment(P > 0.05).Compared with the control group,the TP and ALB test values in the treatment group increased significantly.(4)Results of postoperative complications during hospitalization,subjective comprehensive nutritional SGA evaluation and ADL score: after statistical analysis,there was significant difference between the two groups in terms of postoperative complications during hospitalization(P=0.043 < 0.05),and the overall incidence rate of the control group was higher than that of the treatment group and there were many kinds of complications.There was no statistically significant difference in the evaluation of subjective comprehensive nutrition SGA between the two groups(P=0.273 > 0.05).compared with the treatment group,the control group had more moderate or severe cases.ADL scores of the two groups after 3 months of discharge showed no significant difference(P=0.446 >0.05).Both scores indicated that they were dysfunction and needed more help,and the increase of scores in the treatment group was more obvious than that in the control group.Conclusion:1.Buxue-Yushang Granules can improve the mental fatigue of hypoalbuminemia(deficiency of both qi and blood)after hip fracture surgery in the elderly,who were with a series of symptoms,such as being exhausted,inactive and speechless,spontaneous perspiration,pale or sallow complexion,pale color of lips,eyelids and paws,tepid or shiny limb edema and so on.The curative effect of Buxue-Yushang Granules combined with oral nutrition was superior to that of simply oral nutrition.2.During hospitalization,the incidence of postoperative complications was lower in the treatment group than in the control group,and the number of patients without postoperative complications was higher in the treatment group than in the control group.3.There was no statistical difference in disease efficacy,subjective comprehensive nutritional SGA evaluation and ADL score between the two groups during treatment.4.Buxue-Yushang Granules have no reports of serious adverse effects and toxic or side effects during clinical trials,and are exemplified to be a safe and effective therapeutic drug.
