BACKGROUND:Due to the specialty of the medical industry,the physicians are not only the providers of medical service but also the decision-makers of medical service behavior.Different physician's remuneration design will drive doctors to have different medical service behaviors.The existing allocation methods of performance remuneration do not fully reflect the physician's labor value.One of the goals of the new medical reform is to mobilize the enthusiasm of the medical staff,and the key to mobilize the enthusiasm lies in the reasonable calculation of the remuneration for medical services.The allocation of remuneration based on the labor value can make the remuneration implementing to every diagnosis and treatment project and link to the labor value.Therefore,it is necessary to develop a relative value scale reflecting the physician's labor value and explore a scientific and reasonable distribution mode of physician performance remuneration.PURPOSE:Developed a related value scale based on physician's labor value of radiology projects,calculate the new performance remuneration by the relative value scale,and explore a kind of performance distribution mode which can reflect the physician's labor value in the radiology medical department.METHODS:documentary study,expert value evaluation method,expert consultation method,Standard comparison method1?The development of a related value scale based on physician's labor value of radiology projectsTaking radiology projects in a third class hospital in Guangzhou as an example,we invite 4 professional radiology physician to evaluate relative values by three rounds evaluation by the expert evaluation method.Then it preliminarily form evaluation index results of physician labor services.Then we invite 15 professional radiology experts in some well-known hospital in Guangdong province to judge the reasonableness of evaluation index results of radiology projects and calculate the related value to finally develop the related value scale based on physician's labor value of radiology projects.2?The reasonableness test of the related value scale based on physician's labor value of radiology projectsThis study select some projects from the relative value scale by stratified cluster random sampling method to compare with the price specification of national medical service project in 2012 and the 2015 RBRVS of the United States to observe the rationality and whether the trend is the same in order to further determine the physician labor value of medical radiology projects.3?The application of the relative value scale in the performance distributionWe extract the data of radiology medical department in 2016 in a third class hospital in Guangzhou to calculate the new performance remuneration by the related value scale,which explores a kind of performance remuneration distribution mode which can reflect the value of the physician's labor.RESULTS:1.The related value scale based on physician's labor value of 537 radiology projects was developed.After literature research,three rounds of expert evaluation and expert consultation,we developed 537 items of relative value by the relative value evaluation and calculation and formed a relative value scale based on physician's labor value.2.The rationality test of the relative value scale for medical radiology physicians was carried out.By comparing,the results showed that the relative value scale of this study had same trendency with the 2012 national health service price standard and the 2015 RBRVS of the US.Such comparison results further confirmed the rationality and reliability of the a related value scale based on physician's labor value of radiology projects in this study.3.The related value scale based on physician's labor value of radiology projects was applied in performance distribution.Based on the related value scale based on physician's labor value of radiology projects and combined with the data of imaging medicine in a third class hospital in Guangzhou,we had calculated their performance remuneration and formed the relevant calculation formula in order to reflect the physician's labor value.So in the relative value scale application,each hospital can set up their own the proportion and coefficient according to their actual situation of the hospital,in order to the performance remuneration distribution reflecting physician labor value and the actual working condition of the hospital.CONCLUSIONS:The reliability and rationality of the related value scale based on physician's labor value of radiology projects is high,and the performance distribution model based on the related value scale of radiology projects can truly reflect physician's labor value in the aspect of performance compensation.Due to the limitation of time and conditions,the representation of experts is not reasonable and the sample department and hospital are less.In the following research,the experts in different areas will be added,and the sample department and hospital will be added.The relative value table based on physician 's labor value in different hospital department will be developed,which will lay the foundation for the rational distribution of medical teachers' performance compensation.
