The Study Of Jin's Spirit Regulation Method In Treating Primary Dysmenorrhea

Objective:Dysmenorrhea refers to the menstrual cramps with abdominal pain,tenesmus,backache,sweating,vomiting discomfort and severe symptoms for which can affect sleep or even unable to carry out daily activities.The dysmenorrhea can be divided into two categories.They are Primary Dysmenorrhea and Secondary Dysmenorrhea.The Primary Dysmenorrhea(PD)is a dysmenorrhea with no organic lesion in reproductive organs.It often occurs within 1 or 2 years after menarche,which accounts for more than 90% of dysmenorrhea.Acupuncture treatment of dysmenorrhea can remove obstructions so as to relieve pain.It also has good therapeutic effect and highly recommended by doctors.This study is to observe the effect of JIN's spirit regulation of methodology for the Primary Dysmenorrhea to patients with serum estradiol and progesterone.Its clinical efficacy in explore of therapeutic effect and provides clinical basis for acupuncture treatment of the Primary Dysmenorrhea.Methodology:According to the minimum sample size of 60 cases and the budgetary rate of 10%,the total number of 66 patients were treated.As stated by the random number generator and the factors base on SPSS17.0.The patients were randomly divided into two groups: They were the spirit regulating group and the general acupuncture group.66 patients with primary dysmenorrhea who met the diagnostic criteria that there were enrolled,33 cases in each group.The spirit regulating group was treated by regulating acupuncture.The main points selected were Sishencong,Dingshen needles,Intelligent three needles,Siguan points and Three-yin points.Additional points of Qimen(LR14)and Xuehai(SP10)were apply to those patients with Qi stagnation and blood stasis.Those with inner cold of Yang Deficiency are treated with Qihai(RN6)and Mingmen(DU4).Those with Qi and blood weakness are treated with Qihai(RN6)and Zusanli(ST36).Those with dampness and blood stagnating are treated with Ciliao(BL32)and Diji(SP8).Those with dampness and hotness obstruction are treated with Yinlingquan(SP9),Zhongji(RN3)and Xuehai(SP10).Those with liver and kidney deficiency are treated with Neiguan(PC6)and Zusanli(ST36).Different skill of inserting method on needling were used.Such as needle operated in slow motion,rapid and dart thrusting,lifting thrust reinforcing and the diarrhea reducing.It takes 45 minutes for each treatment.A periodical of 15 minutes using the reinforcing thrusting method and takes four times in diarrheas.Sishencong and Three-yin needles should be warmed of moxibustion for each 20 minutes.The general acupuncture group was treated with conventional acupuncture.The main points selected are Ciliao(BL32),Zhongji(RN3),Diji(SP8),Sanyinjiao(SP6)and Shiqizhui(EX-B7).Those patients with Qi stagnation and blood stasis are treated with addition points Xuehai(SP10)and Taichong(LR3).Those with inner cold of Yang Deficiency are treated with additional points : Qihai(RN6)and Mingmen(DU4).Those with Qi and blood weakness are treated with additional point Qihai(RN6)and Xuehai(SP10).Those with dampness and blood stagnating are treated in additional points Guanyuan(RN4)and Guilai(ST29).Those with dampness and hotness obstruction are treated in additional with Yinlingquan(SP9),Zhongji(RN3)and Xuehai(SP10).Those with liver and kidney deficiency are treated with Shenshu(BL23)and Taixi(KI3).The method for needle injection and the time of treatment was the same as the spirit regulating group.The general acupuncture group was not treated by moxibustion.The curing cycle to both groups were carried out one week before coming of menstrual and an every other day treatment was carried out at the first week.There has no treatment during the menstrual period.The treatment will be continued by two times per week after menstrual.If the dysmenorrhea was serious,we can acupuncture Ciliao with mild moxibustion.It was continuously treating for 3 menstrual cycles.In evaluate the dysmenorrheal symptoms in levels,we can sort out the difference between two groups.At the same time,the levels of serum in E2 and P were also observed.The clinical data for the groups were compared as well as datas were collated and statistically analyzed.Results:1.The two groups of patients were selected with the same background and common information which including age,menarche age,disease duration,and fulfill the HMAM,HMDM and TCM syndrome type.There were no significant difference(P>0.05)between the groups before treatment such as Dysmenorrhea symptom,VAS,E2 serum and P level.Therefore,the selected patients is satisfied the basic requirement and the result is reliable and comparable.2.In comparison the result of the groups.The level of dysmenorrhea symptom,VAS,serum of E2 and P was statistically the significant(P<0.05).It was indicating that two groups can effectively alleviate dysmenorrhea in order to a certain extent and regulate the secretion of the body function and levels of E2 and P.The result after treatment to both groups are fulfill the symptoms in pain score and VAS score,and which satisfied the statistically significant(P<0.05).The efficiency of alleviating dysmenorrhea with the spirit regulating group was better than the general acupuncture group.The serum levels of E2 after the treatment were significantly different from those before the treatment(P<0.05).However,there was no significant difference in serum level of P between the groups at the second cycle of treatment(P>0.05),However there was a significant difference in serum level of P between the groups at the third cycle after treatment(P<0.05).The result indicates that both groups has a regulating effect on E2 and P,and the regulating effect of the spirit regulating group is better than the general acupuncture group.The clinical curative effect,the spirit regulating group was among 31 cases,which as a result of 6 cases were cured,17 cases were markedly effective,6 cases were effective,2 cases were ineffective and the total efficiency of 93.5%.The general acupuncture group was among 30 cases,0 cases were cured,9 cases markedly effective,14 cases effective,7 cases ineffective,the total efficiency of 76.7%,the difference was statistically significant(P<0.05)that metality group was better than the ordinary acupuncture group.ConclusionJin's spirit regulation method was improving the primary dysmenorrhea.It has an obvious curative effect and better than the conventional acupuncture with better dysmenorrhea symptom score and VAS score.They have modulatory role in patients with estradiol and progesterone and the regulating function is observed better than the conventional acupuncture.
