Objectiveby observing the clinical efficacy of warm needle treating primary dysmenorrhea of qi stagnation and blood stasis type, to investigate the possible mechanism of warm needle treating primary dysmenorrhea, to provide theoretical foundation for clinical practice of treating primary dysmenorrhea with acumoxa.MethodCollect patients diagnosed as primary dysmenorrhea of qi stagnation and blood stasis from acupuncture and gynecology department of the first affiliated hospital of GuanGzhou University of TCM between january 2010 to january 2011,72 cases in all. Devide into two groups at random:treatment group 36 cases and control group 36 cases. Control group:take TianQi dysmenorrhea capsuleorally(ingredient:SanQi, WuLingZhi,PuHuang, YanHuSuo, ChuanXiong, MuXiang , XiaoHuiXiang, BingPian)4 pills each time, three times a day, begin to take the medicine three days before menstruation, stop at the third day of menstruation, treatment group:acupoints selection:bilateral lumbar JiaJi points (0.5 cun lateral to the spinous process from the third to the fifth lumbar vertebra),GanShu,PiShu,ShenShu(all bilateral).Manipulation method:patient with dorsal position, vertical insertion on the points above after regular disfection, all use twirling and lifting-thrusting neutral supplementation and draining method, after abtaining qi, put on 1-2cm long moxa stick and ignite, put a 2.5cm*2.5cm thin paper board on the skin under the needle, to prevent scald caused by the falling of ash, waiting for burnout of the moxa sticks, needle retention for 30 minutes. Period of treatment:begin 3 days before menstruation, once each day, until the pain disappear, every menstrual period defined as a treatment course,3 periods for treatment in all. Observe levels of pain scale and score condition of patients before the treatment and after every treatment course to value the clinical efficacy.Result(1) Before the treatment, there is no significant defference between the tw o groups in age, course of disease, level for dysmenorrhea, it is comparabl e.(2) After 3 courses of treatment, symptom of dysmenorrhea improved a lot in both groups, in control group,6 cases healed, obvious effective 12 cases, effective 8 cases.no effect 10 cases, total effective rate is 72.2%. in tr eatment group,13 cases healed, obvious effective 17 cases, effective 4 case s, no effect 2 cases, total effective rate is 94.4%. The curative effect of treatment group is obviously better than that of control group.(3) After the first treatment course, compared with before, level of dysmemo rrhea improved obviously, scores are both decreased compared with before, i t has statistical significance(P<0.05), there is no significant defference in scores between the two groups(P>0.05). After the second treatment cou rse, compared with before the treatment, level of dysmemorrhea improved obv iously for both groups, there is significant defference(P<0.05), compared b etween the two groups, dysmenorrhea improvement of treatment group is bett er than that of control group(P<0.05). After the third treatment course, co mpared with before the treatment, level of dysmemorrhea improved obviously for both groups, there is significant defference(P<0.01), compared between the two groups, dysmenorrhea improvement of treatment group is better tha n that of control group(P<0.05).ConlusionWarm needle can alleviate or remove symptoms of patients with dysmenorrhea effectively, no side effects at the same time, it is a effective method to treat primary dysmenorrhea of qi stagnation and blood stasis, and worthy of made more widespread in clinical practice and being reseached further.
