Clinical Application Of "Kite" Flap Pedicled With Orbicularis Oculi Muscle In The Repair Of Eyelid Defect

Object:To investigate the application and therapeutic effect of “kite” flap pedicled with orbicularis oculi muscle for eyelid defect.Method:Between July 2010 and June 2017,21 patients aged from 40 to 70 years old in our department who were reconstructed eyelid with “kite” flap pedicled with orbicularis oculi muscle in this study.The age range was 40 to 70 years old,the average age was 57.9 years old,6 cases were male(29%),12 cases were female(71%),13 cases were left eye,and 8 cases were right eye.Lesions: 4 cases of upper eyelid,17 cases of lower eyelid,all involving the anterior eyelid.The defect area was 1.3cm × 1.0cm ~ 2.5cm × 2.3cm in the upper eyelids,and 1.5cm × 1.5cm ~ 3.5cm × 3.0cm in the lower eyelids.On the side of the defect area,a "Kite" flap designed with orbicularis oculi muscles is used to repair secondary defects and achieve both clinical and cosmetic effects.Follow-up from 1~6 months after operation was performed to evaluate the postoperative effects of orbital eyelid closure,eyelid eversion,eyelid retraction,eye scar,eyelid swelling,eyelid symmetry,malignant tumor recurrence,and patient comfort.Results:21 cases were treated.All the flaps survived completely and no flap necrosis occurred.Follow-up observation from 1 month to 6 months after operation,21 patients had natural eyelid closure,no eyelid evacuation,eyelid retraction,trichiasis,good bilateral symmetry,no scars,no eyelid swelling,and no recurrence of orbital skin malignancy.No discomfort.In 20 patients,the shape of the eyelids was good,and 1 case showed slight deformation of the internal malleolus,but did not affect the overall shape of the eyelids.Conclusion:1.The "Kite" flap pedicled with the orbicularis oculi muscle can be applied to the repair of moderate defects in the anterior layer of the eyelid.It has both function and aesthetics result,and has high patient satisfaction.2.The secondary defect after enlarged resection of orbital skin malignancy is suitable for clinical promotion of the "kite" flap pedicled with the orbicularis oculi muscle.
