BackgroundThe Common cold is the most common type of acute upper respiratory infection in children.It is a self-limited disease,and there is no specific medicine in modern medical treatment,only for symptomatic treatment.However,the current treatment of the common cold in clinic is blind,repeated and inappropriated combination of drugs,and the frequently use of antipyretics and antibiotics is a potential danger to children's health.Chinese medicine has unique advantages in the treatment of this disease,but it belongs to the stage of experience identification,and the treatment methods are varied and the process is not standardized.With the reform of the medical system in China,the medical insurance payment system is transformed into a single disease direction,improving the quality of medical care,and the standardization and standardization of clinical diagnosis and treatment is the only way for medical treatment.In this case,The clinical pathway as a standardized treatment mode is a worthwhile work method,it can rise the standard medical behavior,reducing the randomness,improving the medical quality,reducing the medical cost.However,the current research in this area is still in the initial stage and lacks strong clinical evidence supports.Therefore,it is necessary to carry out prospective clinical studies on the basis of reasonable clinical pathway to understand the advantages of clinical pathway of common cold wind and fever.ObjectiveThe quality Control Department of the first affiliated Hospital of Guangzhou University of traditional Chinese Medicine,in exploring the innovative project of diagnosis and treatment mode,wants to gradually realize the quality control of single disease in outpatient clinic by constructing the information platform of outpatient clinical pathway,so as to lighten the daily work burden of clinical medical personnel.At the same time,we hope to standardize the process of diagnosis and treatment by the means of clinical pathway,strengthening the management of medical quality,reducing the unreasonable increase of medical expenses,and improving the patients' satisfaction.The Common cold as a common disease in the pediatric department belongs to the category of " exogenous fever " in traditional Chinese medicine,and the traditional Chinese medicine treats the disease from the syndrome differentiation system to the treatment method have rich experiences and have achieved satisfactory results in clinical practice.clinical pathway in outpatient clinic,To preliminarily explore the advantages of TCM clinical pathway in clinical efficacy,economic benefits,patient satisfaction and so on in outpatient clinics with external fever,wind and heat violations.It provides some data support for further expanding the feasibility of implementing outpatient clinic clinical pathway diseases in our hospital.MethodsReferring to the State Administration of traditional Chinese Medicine "Clinical Pathway of external Fever outpatient Clinic" and the special diagnosis and treatment scheme of Pediatrics external Fever in Guangzhou University of traditional Chinese Medicine,By distributing the proportion of 60 outpatients with exogenous fever(common cold)syndrome to three doctors,the doctors were randomly assigned to the path-group and non-path-group according to the single and even number.The path group was treated by the traditional Chinese medicine clinical pathway mode,the non-path group was treated by routine diagnosis and treatment,and the clinical effects of the two groups were observed.The clinical symptoms,symptoms integral of traditional Chinese medicine,and the recovery of body temperature were compared and analyzed before and after treatment.Objective to evaluate the clinical effect of syndrome differentiation of traditional Chinese medicine(TCM)on common cold.ResultsIn the outpatient treatment time,the average number of days in the route group was 2.9494 ± 0.6513 days,the average number of days in the non-route group was 3.7467 ± 0.861 days,there was significant difference between them(P = 0.0186).The average out-of-patient withdrawal time of the route group was 23.5667 ± 16.1962 hours.The average out-out time of the non-route group was 39.33 ± 17.4361 hours,and the withdrawal rate of the route group was higher than that of the conventional group(P = 0.0191,P < 0.05).There was no significant difference between the two groups before and after treatment.There was no significant difference between the two groups after treatment(P < 0.05).It was suggested that the two groups had good curative effect in improving cough,nasal obstruction,fever and nasal dischargeIn the evaluation of comprehensive curative effect,there were 30 cases in the route group,2 cases were cured,21 cases were effective,7 cases were markedly effective,0 cases were ineffective,30 cases were cured,1 case was cured,15 cases were effective and 14 cases were ineffective.There was no statistical significance between them(P < 0.05).In terms of economic benefits : the daily average cost of the route group was 30.2209 ± 7.1847,the daily average cost of the non-path group was 31.3062 ± 10.1907 yuan,there was no statistical significance between them(P >0.05).The total cost was 87.6703 ± 13.81,the non-path group was 108.2965 ± 19.9790 yuan,there was a significant difference between them(P <0.05).Service satisfaction level : The route group was better than the non-route group in the treatment course,the cost and the clinical curative effect,and had statistical significance(P < 0.05)ConclusionThe clinical pathway of external fever(common cold)syndrome can reduce fever,reducing outpatient treatment days,improving clinical efficacy,reduce medical expenses,and improving patients undefined satisfaction.
