ObjectiveTo Observe the Clinical effect of Time-selective extracorporeal shock wave combined with oral traditional Chinese medicine for early and middle stage femoral head necrosis.MethodsSelected from during April 2016 to August 2016 in linyi city people's hospital of specialized subject,a total of 50 patients with 68 hips of femoral head who met the inclusion criteria were randomly divided into two groups: the observation group(Time-selective extracorporeal shock wave combined with oral Huoxue Yanggu decotion)and the control group(other time extracorporeal shock wave combined with oral Huoxue Yanggu decotion).The observation group of 30 patients(42 hips),gender distribution(20 male and female 10 cases,ARC0 stage distribution: phase I 4 hips,stage II 30 hip,phase III 8 hips.Control group 20 patients(26 hips): gender distribution,including 13 cases of male,female 7 cases,ARC0 stage distribution: phase I 3 hips,18 hips phaseII,phase III 5 hip.Observation group(Time-selective extracorporeal shock wave combined with oral Huoxue Yanggu decotion)treatment method is:in the period of the day from 5 p.m.to 7 p.m,the kidney channel by the booming period,the patient adopted lateral supine position,the affected side was on the upper side,the maximum flexion of hip and knee joint,choose the midpoint of anterior superior spine and greater trochanter of femur,the outer 1/3 points of line of posterior superior iliac spine and greater trochanter of femur,and the midpoint of sciatic tuberosity and great trochanter of femur,as hit points,to the direction of the femoral head hit 1000 times each,with the impact point of pain for the degree,according to the patient's situation,each course of shock 5-7 times.After shock wave therapy,health care workers must guide the patients suffering from the quadriceps muscle and lateral contraction practice and hip flexion,internal and external screw,adduction,outreach activities,etc.Shock wave impact 3 period of treatment,1 month between each treatment.Impact after three months,patients with unilateral crutch,one side to avoid weight;Patients with bilateral advice to stay in bed,and without load;Gradually began to load in 3 months.Controlled way(other time blast + huoxue keep bone soup)method is: in renal filled the fill time(every 17 to 19 points),any other time line shock wave therapy,shock wave treatment with timing shock wave treatment.Invigorate the circulation of a bone soup of drugs: corydalis tuber 10 g,radix circumvent 10 g,safflower carthamus 10 g,Angelica sinensis 10 g,Ligusticum wallichii 10 g,radix dipsaci 15 g,scorpion 10 g,rhizoma drynariae 15g?achyranthes root 6g,garden balsam stem 10 g,eucommia ulmoides 15 g,radix angelicae pubescentis 15 g,liquorice 6g.According to the characteristics of traditional Chinese medicine listening,asking and,dialectical add and subtract drugs from the above,the decoction 2 times,filter juice against the 2 points.1 in the morning and after dinner.30 day for a course of treatment.After a period of treatment,upper part,after dialectical add and subtract,herbal research into powder,water flooding into pills,three times a day,every time 6 g,warm water after a meal,take 5 months.Compare two groups before and after treatment in patients with Harris score,dykes,X-ray performance;And through the blood routine,routine urine and kidney function,testing the safety of the treatment.Two groups of the recorded data using SPSSI7.0 for statistical processing.ResultsBefore treatment the observation group and control group patients gender,age cause,necrosis stage(P> 0.05),no statistical difference.After treatment observation group and control group patients' Harris hip score,fine rate,imaging findings of hip joint were significantly improved compared with those before treatment(P<0.05),the Harris score,excellent and good rate and imaging findings of the observation group were better than those of the control group(P<0.05).During the treatment,there were no special abnormalities in blood routine,urine routine,liver and kidney function in the two groups.ConclusionShock wave treatment of femoral head necrosis during renal meridian filling was more effective than that of other times.Huoxue Yanggu decoction has the function of activating blood circulation and regulating qi,dispersing cold and dehumidifying,warming the meridian and strengthening the bone.The combination of the three can promote the differentiation,proliferation and growth of bone tissue,alleviate the symptoms of hip and improve the function of hip joint,and can improve the success rate of hip preservation.
