Objective To explore the clinical efficacy of extracorporeal shock wave combined with hyperbaric oxygen and activating blood circulation to remove stasis drug in treatment of early stage osteonecrosis of the femoral head in adults in order to find out more valuable clinical treatment of early stage osteonecrosis of the femoral head in adults and to provide some theoretical basis for clinical practice.Methods This project was designed to select 30 cases(30 hips)of adult patients with osteonecrosis of the femoral head in Binzhou Medical College Affiliated Hospital from October 2016 to October 2017 for extracorporeal shock wave combined with hyperbaric oxygenation and blood circulation therapy,ARCO There were 17 patients with stage ? and 13 patients with stage ?,of which 19 were alcoholic,8 were steroidal,and 3 were traumatic.The patient's age ranged from 18 to 65 years and was followed up for 3 months.During the study,the affected hips avoided bearing weight within 3 months.All patients received VAS scores and Harris hip scores before treatment.The VAS scores and Harris hip scores were then compared between the first,second,and third treatments.Changes in VAS scores and Harris scores at different time periods and in different phases of the same time period before and after the same stage;color Doppler flow imaging(CDFI)was performed before treatment of the affected limb to calculate the internal diameter of the affected limbs.Arterial blood flow resistance index(RI).After the third course of treatment,the affected limbs were re-performed with CDFI and the flow resistance RI was calculated.The internal diameter of the medial femoral artery was compared between the different stages of the same stage and the different stages of the same period before and after treatment.Changes in blood flow resistance RI;MRI of the pelvic anteroposterior and limbs were taken before and after treatment,and the percentage of the femoral head necrosis area before and after treatment was calculated using the imaging study software to compare the volume of the femoral head and compare phase? and?.Imaging changes of pelvic anteroposterior and limb MRI before and after treatment in patients with different phases of the same period and before and after treatment Volume different stages of osteonecrosis of the femoral head region accounts for a period of time where the percentage change in volume,and finishing the above data were statistically analyzed.Results In 30 patients(30 hips)with early necrosis of femoral head in the adult,after 3 months of follow-up study,it was found that the VAS scores at different time points in the same stage before and after treatment were compared and the difference was statistically significant(P<0.05).There was a statistically significant difference between different stages of the segment(P<0.05);Harris scores at different time periods before and after treatment were compared with each other,and the difference was statistically significant(P<0.05).Differences between different stages of the same time period were different.There was statistical significance(P<0.05);the RI of the ipsilateral femoral head of the affected femoral head at different stages of the same stage before and after treatment was decreased,and the difference was statistically significant(P<0.05).Different stages of the same time period There was no statistically significant difference between the two groups(P>0.05).The volume of femoral head necrosis in the affected area at the same stage before and after treatment decreased as a percentage of the volume of the femoral head,and the difference was statistically significant(P<0.05).There was a statistically significant difference between different stages of the segment(P<0.05).Conclusion Extracorporeal shock waves combined with hyperbaric oxygenation and blood circulation therapy have significant effects on early necrosis of femoral head in adults,and stage I is better than stage II.Triple therapy can significantly improve the patient's joint function and relieve pain,which is of great value in clinical application.
