Objective:To study the curative effect of Huoxue Yanggu Decoction on non-traumatic osteonecrosis of the femoral head and to provide more effective clinical treatment for the femoral head necrosis. Methods:A total of 60 patients with femoral head non-traumatic necrosis were treated with a prospective randomized controlled study method from February 2015 to February 2016 in the Luoyang Orthopedic Hospital hip injury treatment center(Henan Provincial Orthopedic Hospital). The patients were divided into experimental group and control group according to the random number table method with 30 cases for each group. The two groups were treated with bone graft fibular support surgery.Based on conventional treatment, the experimental group received oral administration of blood circulation soup. The Harris score,VAS pain scores, and overall imaging evaluation were performed at the following time points:before the operation,3,6,9 and 12 months after the operation. The clinical data were analyzed by using SPSS20.0 statistical software.Results:27cases(36 hips)in the treatment group and 26cases(34 hips)in the control group were followed up for 12 months. The gender, age, etiology, course of disease, necrosis,ARCO stage, preoperative VAS score and Harris score of the two groups were compared. The difference was not statistically significant(P>0.05),and the data of two groups are comparable. There was no significant difference between the two groups in operation time and postoperative blood loss(P>0.05).The Harris scores of the two groups were significantly higher than those before surgery(P<0.05)at 3 months after operation. There was no significant difference between the two groups(P>0.05).The difference between the two groups was statistically significant(P<0.05)at 6,9,12 months after operation. And the interaction between the two groups was statistically significant(P<0.05).There was no significant difference in VAS score between the two groups at 3 months(P>0.05).There was significant difference between the two groups(P>0.05)of VAS scores at 6,9,12 months after operation. And the interaction between the two groups was statistically significant(P<0.05).Overall imaging evaluation:In the 36 hips of the experimental group, the uneven density of femoral head and cystic lesions were found in one patient with II B phase in the follow-up after 12 months. Two patients' condition progressed from stage ?C to stage ?A,but the collapse was less than 2mm for them.No lesions were found in the remaining 33 hips.Obvious osteogenesis was found in the femoral head bone graft area, and the new bone replaces most of the necrotic bone in lesion area According to the overall imaging evaluation criteria,19 hips improved,14 hips stable,3 hips increased. In the 34 hips of the control group, one patient's condition progressed from stage ?A to stage II B,four patients' condition progressed from stage ?B to stage ?C,three patients' condition progressed from stage ?C to stage IIIA,and no significant changes were found in the remaining 26 hips.Newborn bone replaces part of the necrotic bone.According to the overall imaging evaluation criteria,10 hips improved,16 hips stable,8 hips increased. There was significant difference between the two groups(P<0.05).Conclusion:Oral Huoxue Yanggu Decoction can effectively relieve the patients' symptoms after operation, improve their joint function, improve the survival rate of bone graft and further improve the curative effect of bone grafting combinated with fibula support.
