The Study On "Two-dimensional Traction" In Degenerative Lumbar Spinal Stenosis

ObjectiveTo evaluate the effect of two-dimensional traction on degenerative lumbar spinal stenosis,and compared clinical observation results with the pelvic traction.To explore the effect of the principle of two-dimensional traction for the treatment of degenerative lumbar spinal stenosis and provide a safe and effective treatment method.Methods60 cases of degenerative lumbar spinal stenosis,in the Foshan Hospital of TCM from April 2017 to January 2018,were randomly divided into the treatment group(two dimensional traction)and control group(the pelvic traction).Two group were given the same conventional basic treatment for 2 weeks,respectively to observe the subjective pain score(VAS),Oswestry disability index(ODI)and the curative effect score with low back pain in Japanese Orthopaedic Association(JOA)before and after treatment.To collect the data for statistical analysis,to compare the observed indexes and evaluate its clinical efficacy.Results1.The effects of two groups of different traction methods on the treatment of degenerative lumbar spinal stenosis were compared by VAS score,ODI index and JOA score.The results showed that there was no significant difference between the two groups of VAS scores,ODI index and JOA score before treatment(P > 0.05).2.The total effective rate of the two-dimensional traction group was 93.33%,which was higher than that of the bedside pelvic traction control group(83.33%),with statistical significance(P < 0.05).3.2 weeks after treatment,there were significant differences in the three indexes between the two experimental group and the control group(P < 0.05),indicating that two dimensional traction improved the clinical symptoms of lumbar spinal stenosis than bedside pelvic traction.ConclusionThe main characteristics of degenerative lumbar spinal stenosis are intermittent claudication and pain.The focus of treatment is to relieve symptoms.Two-dimensional traction refers to the simultaneous movement of the prone pelvic traction and the abduction traction of the lower extremities.It can adjust the lumbar sequence abnormalities caused by the disorder of the lumbosacral joint disorder.In the process of traction,the muscles of the back include the erect spinal muscle and the psoas muscle relieve the inflammation of the fascia of the back and the fascia of the latissimus latus,change the original tension and relaxation state of different ligaments.It also adjusts the curvature of the lumbar spine by the synergistic action of the muscles,joints and ligaments.It makes a small shift between the vertebral body makes the spinal canal widen relatively,relieving the symptoms of nerve compression and alleviating the pain.Two-dimensional traction is simple,safe and effective.It is worth popularizing in clinical practice.
