| Objective :It is to analyze the differentiating and diagnosing role of modified traditional Chinese medicine(TCM)parturition-hastening beverage in pregnancy and to investigate the efficacy and safety of its combination with oxytocin in clinically promoting cervical ripening.Methods: A retrospective analysis was conducted on the parturients in their late trimester of pregnancy admitted for treatment in Zhangjiagang City Hospital of Traditional Chinese Medicine from October 2016 to September 2017.Trained attending physicians assessed the subjects of singleton pregnancy.In accordance with certain inclusion,exclusion and withdrawal criteria,97 subjects were enrolled.According to the principle of randomization and willingness of the subjects,47 subjects were included in the study group and 50 in the control group.In the study group,subjects took orally the modified TCM parturition-hastening beverage and received low-dose intravenous drip of oxytocin(1.25 U).In the control group,subjects received intravenous drip of 2.5U oxytocin only.Statistical analysis was conducted on the age,BMI index,gestation,parity,cervical Bishop score,estimated fetal weight(EFW),indication of labor induction,effective rate of cervical ripening,time elapsed from administration to in-labor(min),time elapsed from in-labor to cervical dilation of 3 cm(min)and adverse event rates of the subjects in both groups.Results:(1)For partirients with labor induction by methods respectively,no significant difference was identified in the clinical features and cervical Bishop score between them(P>0.05).No significant difference was identified in age,BMI index,gestation,proportion of primipara,cervical Bishop score,EFW and indication for labor induction(P>0.05).(2)The cervical Bishop score after administration increased significantly in both groups compared with that before administration.Comparisons between the two groups showed the Bishop score in the study group increased more remarkably.There is significant difference between the two groups(P<0.05).In the study group,the in-labor rate within 12 hours after administration,significantly effective rate within 24 hours after administration,effective rate within 72 hours after administration were 14.89%,42.55% and 68.09%,respectively.In the control group,the in-labor rate within 12 hours after administration,significantly effective rate within 24 hours after administration,effective rate within 72 hours after administration were 8%,24% and 54% respectively.Compared with the control group,the study group has faster promotion of cervical ripening,shorter time elapsed and higher scores.There is significant difference between the two groups(P < 0.05).(3)Comparison of the total effective rate of promoting cervical ripening in the groups;the total effective rate of promoting cervical ripening in the study group was 91%,which was significantly higher than that in the control group(P < 0.05).The difference between the two groups is significant(P<0.05).(4)Compared with the control group,the time elapsed from administration to in-labor(min)and the time elapsed from in-labor to cervical dilation of 3 cm(min)in the study group were significantly shorter than those in the control group.There were significant differences between the two groups(P< 0.05).(5)The comparison of adverse reactions between study group and control group;in the study group,two subjects underwent mild diarrhea and the incidence of forced uterine contraction and abnormal fetal heart rate were lower than those in the control group.No significant difference was identified(P > 0.05).The success rate of promoting cervical ripening in the control group was relatively lower and the cesarean rate was higher accordingly.Conclusion: 1.The modified TCM parturition-hastening beverage promotes cervical ripening with a high success rate.The formula focuses on promoting blood circulation to remove blood stasis and cervical softening.Radix codonopsis is added to tonify Qi and assist labor;Poria cocos added calms the mind and relieves the nervousness of pregnant women.It therefore lets out the toxins without hurting the body and combines tonification and purgation.The combination of modified TCM parturition-hastening beverage and small-dose oxytocin brings faster promotion of cervical ripening with shorter time and higher score.The onset of labor is ignited effectively in time.2.The modified TCM parturition-hastening beverage is safer compared with oxytocin.No obvious adverse reactions occur during administration;excessive uterine contraction,tetanic contraction of uterus and abnormal fetal heart rate are rare.It also increases the cervical Bishop score,promotes the onset of labor,relieves the fatigue of parturient,and reduces postpartum hemorrhage.It also has a high success rate of promoting cervix ripening,lower rate of cesarean section,reduced opportunity for operations,and lower risks for both the mother and the infant.3.The modified TCM parturition-hastening beverage is relatively mild and highly acceptable for parturients.The combination of TCM and low-dose oxytocin does not cause selfconscious uterine contraction during intravenous drip,but can accelerates cervical softening and reach the expected effect for parturients.Compared with other methods of delivery expedition and labor induction,it is conductive to relief of the mental stress of pregnant women and efficient and successful delivery.This study reflects the concept of holism in traditional Chinese medicine.It regulates the functions of internal organs as a whole,helps strengthen the body resistance and eliminate pathogenic factors,and play an effective role mildly.It is convenient to take the modified TCM parturition-hastening beverage without side effects.Patients show high compliance.It also costs less,shortens the days of hospital stay and reduces medical expense.It is a low-cost,safe and effective method for labor induction that is worthy of popularizing. |
