Objectives To compare and evaluation the effect and safety of COOK balloon and Dinoprostone on cervical ripening and induction of labor in full-term pregnant women and provide a basis for clinical choice of induced labor.Methods 236 pregnant women at the age of 37 weeks to 42 weeks,who were expecting induction of labor,were enrolled at North China University of Science and Technology Affiliated Hospital to be delivered from November 2016 to November 2017.According to the combination of disease and voluntary principles,different methods of induction of labor were selected.According to the mode of induction of labor,115 patients were divided into COOK balloon group and 121 in Dinoprostone group.A self-made observation table was used to collect the age,height,prenatal body weight,gestational week,gravidity and parity history of the study subjects through asking patients or electronic cases.According to labor progress,timely record of two groups of pregnant women progress of labor process,induction of induction of labor,cervix Bishop score before and after induction of labor,drug or balloon withdrawal indications,induction of adverse reactions(abnormal fetal heart rate,fever,excessive uterine stimulation,etc.),indications for cesarean section,mode of delivery,cervical laceration,precipitate labour,postpartum hemorrhage and other maternal complications,neonatal body weight,neonatal umbilical arterial blood gas analysis,neonatal 1 minute Apgar score,and neonatal admission to intensive care unit.Record hospital days and all hospitalization costs;we were statistically analyzed by SPSS17.0,measurement data using t test,count data using ?2 test or Fisher's Exact method.For data with uneven variance,check the logarithmic transformation.P<0.05 regareded as statistically significant.Results 1 the average age of pregnant women,progestational weeks before induction,prenatal body weight,height,prenatal body mass index,and the proportion of pregnant women in the two groups were not significantly different(P> 0.05).Indications of induction of labor,COOK balloon group gestational hypertension disease were significantly higher than the Dinoprostone group,the difference was statistically significant(P<0.05),the other differences were not statistically significant(P> 0.05).2 After induction of labor,the cervical score of COOK balloon group increased to(6.98±1.99)points on average,and the cervical score of Dinoprostone group increased to(7.18±1.93)points on average,the difference was not statistically significant(P>0.05).However,the cervical scores in both groups were significantly higher than those before induction,with an average score of more than 3 points,which was significantly different from those before induction(P<0.05).The total effective rate of promoting cervical ripening was above 80%,and the total effective rate of induction of labor was above 65%,but there was no significant difference between the two groups(P>0.05).3 The rate of fetal heart abnormalities,uterine hyperstimulation,and fever during the induction process of Dinoprostone was high,the difference was statistically significant(P <0.05).There were no cases of excess uterine stimulation in the COOK balloon group.There was no difference in the indications for diverting cesarean section between the two groups(P>0.05).4 COOK balloon group intervention to labor time,intervention to delivery time was significantly longer than Dinoprostone group,and the difference was statistically significant(P<0.05).The total labor,the first stage of labor,active phase,the second stage of labor,the difference was not statistically significant(P>0.05).5 Vaginal delivery rate(74.8%/72.7%)and cesarean section rate(25.2%/27.3%)in both groups were not statistically significant(P>0.05).The 48-hour delivery rate of the Dinoprostone group was significantly less than the COOK balloon group(P<0.05).The number of urgencies,the number of cervical lacerations,the number of assisted vaginal delivery and the number of postpartum hemorrhage were higher in Dinoprostone group than those in COOK balloon group,but the differences were not statistically significant(P>0.05).Neonatal 1 minute Apgar score,umbilical artery PH value,the difference was not statistically significant(P> 0.05).6 The number of hospital stays in the Xinpubeisheng group was not statistically significant(P>0.05),but the hospitalization expenditure was significantly lower than the COOK balloon group(P<0.05).Conclusions The results obtained through the class test are as follows: 1 COOK balloon and Dinoprostone are both effective and safe for cervical ripening and induced labor.2 The time taken by Dinoprostone to produce labor was significantly shorter than the COOK balloon,but the two had no effect on the total production process.3 High incidence of uterine overstimulation and abnormal fetal heart rate during induction of labor in Dinoprostone.The induction of COOK balloon is relatively mild,relatively safe,and increases the chance of delivery for pregnant women who cannot tolerate strong contractions.
