Objective:Investigation on that effect of oral vitamin D preparation on immune function and therapeutic effect of PTB patients with decrease in25-hydroxy vitamin D.Methods:We enrolled 35 first-treated PTB patients with normal serum25-hydroxy vitamin D(Group A)and 70 first-treated PTB patients with25-hydroxy vitamin D deficient(<75nmol/L)(Group B).The patients in Group B were randomly divided as control group(B1)and treatment group(B2).The patients in group A were given common anti-TB drugs as 2HRZE/7HRE(If the patient had the risks for liver injury,older patients,or chronic diseases,2HLZ+Lfx/7HLE were given).Routine blood test,biochemical test,CD4~+T cell numbers,and time for TB smear negative translation were tested at 1 week,2week,1m,2m,6m and at the end of anti-TB treatment;,Radiographic improvement was 1m,2m,6m and at the end of anti-TB treatment.The patients in B2 were given anti-TB treatment and dose-titered oral Vitamin D,while B1patients were just given anti-TB treatment.Routine blood test,biochemical test,CD4~+T cell numbers,time for TB smear negative translation,and radiographic improvement were tested at 1 week,2 week,1m,2m,6m and at the end of anti-TB treatment.Results:1.The levels of serum Vitamin D had no effect on the overall prognosis of tuberculosis(?2=0.873,P=0.350>0.05);2.The number of CD4~+T cell was increased in B2 group after Vitamin D supplement(t=-4.345,P=0.000<0.005);3.Supplement vitamin D,maintaining the normal range of vitamin D level,can shorten the time of TB smear negative translation(t_D=5.90,P=0.00<0.05)and radiographic improvement in B2 group(t_D=5.08,P=0.00<0.05)?Conclusion:Vitamin D supplement in PTB patients with vitamin D levels dropped can't significantly improve the curerate,but can increase the number of CD4~+T cell,shorten the time for TB smear negative translation,and speed up the imaging absorption of diseases.Vitamin D can be applied to patients with vitamin D deficient,so as to control infection as soon as possible,and reduce the harm to public health.At the same time,the imaging absorption of diseases can reduce the effect of lung damage on lung function and improve the quality of life.
