Objective:To compare the short-term and long-term efficacy of microvascular decompression for typical and atypical trigeminal neuralgia patients by meta analysis,and to analyze the factors influencing postoperative efficacy.Methods:We searched the database in Chinese and English,strictly screened the literature,and evaluated the quality of the included studies.We used Review Manager 5.3 to analyze the clinical characteristics,postoperative efficacy and the factors influencing the treatment effect of typical and atypical patients.Results:Patients with typical trigeminal neuralgia were more likely to affect a single branch of the trigeminal nerve and to have a family history.Patients with typical trigeminal neuralgia were more likely to achieve the immediate relief of pain after microvascular decompression,5 year postoperative pain relief and 5 year postoperative pain recovery.Patients with atypical trigeminal neuralgia have a higher recurrence rate.For typical trigeminal neuralgia patients,the positive predictive factors were:trigger points before operation,unilateral facial pain before operation,memory for first time pain before operation,no sensory damage before operation,and no destructive operation before operation.The factors that have a positive predictive effect on the postoperative outcome of atypical trigeminal neuralgia are:no sensory impairment before operation,and no destructive operation before operationConclusion:After microvascular decompression,patients with typical trigeminal neuralgia are more likely to receive pain relief.
