| Objective: To analyze the causes of problems after anterior teeth full crown restoration that will provide references for the treatment of anterior teeth restoration or reconditioning,which is beneficial for improving the effect and success rate of anterior teeth restoration.Method: Five clinical cases of anterior teeth full crown restoration were selected in Dalian Stomatological hospital from October 2015 to December 2017.Meanwhile,the electronic information of 300 patients who had problems of anterior teeth full crown restoration were selected systematiacally.Inclusion Criteria: 1.No systemic disease,no history of drug allergy and no history of smoking.2.Prosthesis position: anterior teeth area.3.Prosthesis types : full crown,post and core crown.4.no dentition defect in anterior teeth area.Exclusion Criteria: Female patients with pregnancy were ruled out.Search method: According to the patients' chief complaint,present history and past history,preliminary information of the problems after restoration were obtained.Then oral examination included teeth,prosthesis,periodontics,occlusion,dentia and soft tissue,meanwhile X-ray,panoramic image or Florida test should be taken for the patients,which were used to analyze the problems of anterior teeth restoration.After having good communications,five patients were carried out comprehensive oral health education and offered the treatment plans that were designed by assessing the requirement and willingness of patients themselves.Transfer therapy was offered if theyunderstood and accepted the treatment process.The intraoral and external photographs,plaster models,Digital Smile Design,the diagnostic wax design and other means were used to make an aesthetic restoration plan,and then diagnostic wax was mocked up for making temporary crowns.Auxiliary diagnostic temporary crowns were used for three months,during this period,the aesthetics,pronunciation and occlusal function of the temporary crowns were evaluated.When biololgy and biodynamics were ensured and the auxiliary diagnostic temporary crowns were approved by the patients,then teeth preparation were done.When the new restorations were finished,the patients were suggested to see for follow-up visits three months later and the effect of treatment would be evaluated.Results: 1.Reasonable restoration designs which combined with their own conditions and requirements were given to the five patients.The aesthetics,occlusal function and pronunciation of the crowns were accepted by the patients after the treatment.Three months later,five patients returned visit,they were satisfied for the new anterior teeth restorations.The restorations made patients more confident than before.2.Problems statistics in 300 cases of anterior teeth full crown restoration as follows: there were 68 male patients and 232 female patients,amount to 915 anterior teeth.138 patients had the symptoms of swollen gums,128 patients had the symptoms of abutments pain,there were 66 patients whose post and core crown had fallen down,there were 38 patients whose crown had fallen down,there were 30 patients whose gingivas had been dyed black,there were 28 patients whose procelain crown had fractured,9 patients were not satisfied for aesthetics of the crowns,there were 3 patients whose post and core crown had broken.Conclusion: 1.It's so significant to analyze the problems of initial restoration reasonably.The similar prombles should be reduced after the related re-treatment,meanwhile the requirements of patients and a reasonable design for the patient should also be considered.2.It was estimated that dentists' treatment design and operation,technicians' manufacturing level,prosthetic quality and patient's oral sense of maintenance were associated with the problems of restoration.3.According to thestatistics of 300 cases,the symptoms of swollen gums and the abutments pain were the main problems after the full crown restoration of anterior teeth,so dentists should pay attention to these problems in the clinic. |
