Objective1.To compare diagnostic value of bone mineral density of the different parts in elderly male type 2 diabetes;2.To investigate the prevalence rate of osteoporosis in elderly male patients with type 2 diabetic of clinic,and analyze the related factors of type 2 diabetic patients with osteoporosis.Methods1.196 patients with elderly male type 2 diabetes from endocrine clinic of our hospital were measured in lumbar spine and femur neck BMD.97 elderly male patients with type 2 diabetes and normal BMD were selected.Dividing into groups by age(60-69?70-79?over 80 years old)and comparing the BMD of left femur neck?right femur neck?lumbar spine(1?2?3?4)of different age groups to find out the part of maximum difference among groups.2.Dividing the 196 patients with elderly male type 2 diabetes into three groups according to the osteoporosis standard of WHO,97 cases with normal BMD?43 cases with osteopenia?56 cases with osteoporosis were included.Calculating the prevalence rate of osteoporosis in type 2 diabetes,and comparing the age?courses of disease,BMI?FPG?PPG?HbA1c?SBP?TG?TC?HDL-C?LDL-C?metabolism index of bone[Ca?P?ALP?PTH?25(OH)D?osteocalcin?P1NP??-CTX?left femur neck BMD]?related medicine.The correlation of these factors and osteoporosis was analyzed by logistic regression analysis.Results1.The BMD of left femur neck is obvious different among groups(P<0.05),and decreases along with the age.The BMD of lumbar spine is similar among groups,and rises after decreases along with the age.2.The prevalence rate of osteoporosis in elderly male patients with type 2 diabetes of clinic was 28.6%.The age?course of disease?BMI?FPG?PPG?HbA1c?P?ALP?osteocalcin??—CTX?left femur neck BMD?TZDs?PPIs?Vitamin D had a statistical significance(P<0.05)between the three groups,but the Ca?PTH?25(OH)D?SBP?PINP?Tg?Tc?HDL-C?LDL-C?other medicine were not different.The age?course of disease?FPG?PPG?HbA1c?P?ALP?TZDs?PPIs drug were negatively related to BMD,the BMI was positively related to BMD.Logistic regression analysis revealed that age?course of disease?BMI?HbAlc?TZDs?PPIs drug were related with osteoporosis.ConclusionLeft femur neck can be regarded as the measuring part with vital diagnostic value for elderly male type 2 diabetes.The advanced age?longer course of disease?lower BMI?higher HbA1c?TZDs?PPIs drug may increase the risk of osteoporosis in elderly male patients with type 2 diabetes.
