Objective1.To explore the effectiveness of plasma-mediated radiofrequency ablation debridement in treatment with postoperative orthopaedic infection.2.To develop the novel plasma surgery system for wound debridement,aim at the deficiencies of the conventional system.3.To verify the safety and effectiveness of the novel plasma surgery system for wound debridement in vitro and vivo experiments.Methods1.16 patients with early postoperative infection of internal fixation for fracture were treated.Conventional plasma surgery system was conducted to debride the foci of infection with retention of internal fixation,and the effectiveness was evaluated.2.The novel plasma surgery system for wound debridement was designed and developed to improve the deficiencies of the conventional system,cooperating with Chengdu Mechan Electronic Technology Co.,Ltd.3.27 vitro infective wound models prepared in the fresh pork skin were divided into 3 groups randomly:the novel plasma surgery system group,the sharp debridement group and the conventional plasma surgery system group.The operation time of debridement for each group was evaluated.4.160 vivo infective wound models prepared in 8 minipigs were divided into 4 groups randomly:the conventional plasma surgery system group,the conventional plasma surgery system group,the sharp debridement group and the control group.The time of operation,the healing time,the healing rate,the tissue filling rate,the bacterial quantitative examination and the histological examination of each group were evaluated.Results1.All the 16 patients,followed up 12-36 months after operation,were cured and healed by first intention without recurrence.The deficiencies of the conventional plasma surgery system for debridement were evaluated,such as the inadequacy of the handle,the low efficiency of debridement,and the vulnerability of blockage.2.Cooperating with Chengdu Mechan Electronic Technology Co.,Ltd,we developed the novel plasma surgery system for wound debridement finally.3.There were significant differences between the operation time of three groups.Comparing with the conventional plasma surgery system group,the efficiency of debridement and the vulnerability of blockage of the novel plasma surgery system group were significantly improved.4.Comparing with the conventional plasma surgery system group,the efficiency of debridement and the vulnerability of blockage of the novel plasma surgery system group were significantly improved.Meanwhile,there is no significant difference of the healing time,the healing rate,the tissue filling rate,the bacterial number between the novel plasma surgery system group and the conventional plasma surgery system group.But,the results of both two plasma surgery system groups were better than the sharp debridement group and the control group.ConclusionsComparing with the conventional plasma surgery system,the efficiency of debridement and the vulnerability of blockage have been improved in the novel plasma surgery system for wound debridement,which would offer a new method for the treatment of chronic wounds and represent considerable military,economic and social benefits.
